血清胱抑素C水平与急性脑梗死的相关性研究

     

摘要

Objective To investigate the relationship of acute cerebral infarction (ACI) and serum cystatin C (Cys C) and its influence to the prognosis of ACI.Method Analyzed the history and related biochemical results of one hundred and nine cases with ACI and one hundred and thirteen patients with no history of cerebral infarction,all the subjects were detected serum Cys C levels,patients with ACI were assessed the prognosis by using the mRS scores in 90d after the onset of infarction.Results The serum Cys C level (1.33 ± 0.52mg · L-1) of ACI group increased significantly when compared with the control group 1.10 ± 0.34mg · L-1 (P<0.001);Logistic regression analysis showed that the serum Cys C (OR=3.772 95%,CI 1.60~8.92,P=0.003),hypertension (OR=2.233,95% CI 1.23-4.06,P=0.008),smoking (OR=1.915,95% CI 1.06~3.45 P=0.030),glyeosylated hemoglobin (OR=1.214 95% CI 1.01-1.47,P=0.043) are risk factors for ACI;according to serum Cys C level:less than 0.81 mg · L-1 in 4 patients,0.82-1.70 mg · L-1 in 84 cases,1.71-2.58 mg · L-1 patients with sixteen cases,more than 2.59 mg · L-1 in five cases,these four groups compared with NIHSS score and mRS score,showed that the serum Cys C is independent with the severity and prognosis in 90d of ACI.Conclusion The level of serum Cys C was significantly increased in patients with ACI,and is independently associated with the attack of ACI.%目的 探讨急性脑梗死(ACI)患者血清胱抑素C(Cys C)水平变化情况及其对ACI预后的影响.方法 分析109例ACI患者及1 13例无脑梗死患者病史及相关生化分析结果,全部受试对象完善血清Cys C水平检测,ACI患者均在发病后90d进行mRS评分,评估患者预后.结果 ACI组患者血清Cys C水平(1.33±0.52mg·L-1)与对照组相比(1.10±0.34mg·L-1)明显升高(P<0.001);Logistic回归分析显示分析,Cys C(OR=3.772,95% CI 1.60~8.92,P=0.003),高血压病(OR=2.233,95% CI 1.23~4.06,P=0.008),吸烟(OR=1.915,95% CI 1.06~3.45,P=0.030),高糖化血红蛋白(OR=1.214,95%CI 1.01~1.47,P--0.043)是ACI的危险因素;按血清Cys C水平分为≤0.81mg·L-1患者4例,0.82~1.70mg·L-1患者84例,1.71~2.58mg·L-1患者16例,≥2.59mg·L-1患者5例,4组与NIHSS评分、mRS评分等进行分析显示,血清Cys C与ACI严重程度及90d预后无关.结论 单血清Cvs C水平在ACI患者中升高明显,并与ACI发病独立相关.
