Objective To investigate the effect of bronchoscopy lavage in the treatment of acute exacerbation of bronchiectasis.Methods A retrospective analysis of clinical data was carried out in 56 patients with acute exacerbation of bronchiectasis admitted to our hospital from January 2013 to January 2015,who were divided into control group (25 cases) and observation group (31 cases) according to different therapeutic schemes.Control group was given ambroxol injection 30 mg + 5％ glucose injection 100 ml intravenous drip,2 times per day;on the basis of control group,observation group was given ambroxol injection 60 mg + normal saline 10 ml bronchoscopy instillation,2 times per week;14 d as a course,they all lasted for 2 courses of treatment.Clinical efficacy was assessed,mucus density (MD),mucus volume (MVP),sputum volume,blood oxygen pressure (PaO2),blood oxygen saturation (SaO2) changes were compared between two groups after treatment.Results The effective rate of observation group was 96.77％,which was significantly higher than that of control group (80％)(P＜0.05);MD and MVP scores after treatment of observation group were lower than those of control group,and the sputum volume was less than that of control group (P＜0.05);after treatment,SaO2,PaO2 of observation group were significantly higher than those of control group (P＜0.05).Conclusion Bronchoscopy lavage is of obvious curative effect in treating bronchiectasis exacerbations,which can reduce the amount of expectoration.%目的 探讨纤维支气管镜(纤支镜)灌洗治疗支气管扩张急性加重期的疗效.方法 回顾性分析我院2013年1月至2015年1月56例支气管扩张急性加重期临床资料,根据治疗方案为对照组25例和观察组31例,对照组给予氨溴索注射液30 mg+5％葡萄糖注射液100 ml静滴,2次/d,观察组在对照组基础上给予氨溴索注射液60 mg+生理盐水10 ml行纤支镜灌药,2次/周;均以14d为1个疗程,共治疗2个疗程.评估临床疗效,对比两组治疗后黏液密度(MD)、黏液体积(MVP)、痰量、血氧分压(PaO2)、血氧饱和度(SaO2)变化.结果 观察组治疗有效率为96.77％,显著高于对照组80.00％(P＜0.05)观察组MD、MVP评分均低于对照组,痰量少于对照组(P＜0.05);观察组治疗后PaO2、SaO2均明显高于对照组(P＜0.05).结论 纤支镜灌洗治疗支气管扩张加重期疗效确切,可减少咳痰量.
