危重患者院内安全转运管理程序的构建与效果评价

摘要

目的 探讨构建危重患者在院内安全转运程序,并观察运用效果,为危重患者在院内安全转运程序推广运用提供临床依据.方法 回顾性分析2015年1月至2016年12月本院ICU收治纳入标准的328例危重患者作为研究对象,其中2015年1至12月采用传统方法转运的158例患者为对照组,2016年1至12月采用构建危重患者院内安全转运管理程序并实施的170例患者为实验组.观察两者患者在院内转运所需时间及在转运过程中不良事件的发生率.结果 实验组患者转运时间为(20.74±3.20) min、不良事件发生率为4.71％,明显优于对照组[分别为(30.81±3.54) min、36.71％],差异均有统计学意义(均P＜ 0.05).结论 为ICU危重患者制定出规范的危重患者转运流程,规定转运医护人员资质,集体对患者转运过程中风险进行科学评估,转运过程中所需药品和物品配备齐全,对参与转运医护人员进行情景模拟演练和培训等院内安全转运管理程序,保障了危重患者在院内转运过程中的安全,降低了患者发生转运相关的不良事件,对患者疾病的最终转归有着重要意义.%Objective To explore the construction of safe transport procedure for critical patients at hospital and to observe the application effect,and to provide clinical basis for the promotion and application of safe transport procedure at hospital for critically ill patients.Method From January,2015 to December,2016,328 critically ill patients admitted into ICU at our hospital were retrospectively analyzed;158 patients who were transported by traditional methods from January to December,2015 were set as a control group;and from January to December,2016,170 patients were set as an experimental group.The transport time at hospital and the incidence of adverse events in the process of transport were observed.Results The transport time of the patients was significantly shorter and the incidence of adverse events was significantly lower in the experimental group than in the control group [(20.74 ±3.20) min vs.(30.81 ±3.54) min and 4.71％ vs.36.71％,all P ＜ 0.05].Conclusions The standard process of critical patient transportation was established for ICU critical patients;the qualification of medical personnel was stipulated;and the risks in the process of patients' transportation were evaluated scientifically.The drugs and articles needed in the process of transportation were well equipped.In order to ensure the safety of the critically ill patients during the in-hospital transportation process and reduce the adverse events related to the patients' transportation,the scene simulation exercise and training of the medical personnel involved in the transfer of medical and nursing personnel were carried out at the hospital.All these are significant for the patients' final outcomes.
