2013年我院PIVAS若干不合理医嘱分析及干预

摘要

Objective To reduce the number of irrational prescription and promote clinical rational drug use.Methods The irrational prescriptions were collected from January to December, 2013 on the platform of PIVAS HIS.All irrational prescriptions were statistically classified and analyzed.Then interventions were introduced to solve the relative problems.Results There were 591 irrational prescriptions in 2013, among which the improper selection of solvents accounting for 36.72%, overdose accounting for 16.58%, drug high concentration accounting for 16.07%, drug incompatibility accounting for 14.38%, improper frequency of administration accounting for 12.18% and other accounting for 3.89%.Conclusion Corresponding interventions were used to decrease the existing problems of irrational drug use. Finally, irrational prescriptions were obviously reduced.%目的：减少病区不合理医嘱例数，促进临床用药合理性。方法以PIVAS HIS系统为平台，收集我院PIVAS审方药师在2013－01～2013－12间所记录的不合理医嘱，对其进行分类统计并就有关问题进行干预。结果全年不合理医嘱591例，其中溶媒选择不当占36．72％，超剂量使用占16．58％，给药浓度过高占16．07％，药物配伍不当占14．38％，给药频次不当占12．18％，其他占3．89％。结论针对存在的各种不合理用药问题，我们采取相应措施进行干预，经过干预后不合理医嘱现象明显减少。
