稳心颗粒联合胺碘酮和培哚普利治疗老年阵发性心房颤动的临床效果及安全性

摘要

目的 研究稳心颗粒联合胺碘酮和培哚普利治疗老年阵发性心房颤动的临床效果和安全性.方法 心内科治疗的老年阵发性心房颤动患者116例,随机分为对照组和观察组,每组58例.对照组使用胺碘酮联合培哚普利治疗,观察组有对照组的基础上加用稳心颗粒治疗.比较2组患者治疗后的效果和不良反应等.结果 观察组有效率为90.57％,对照组有效67.27％,2组差异有统计学意义(P＜0.05).2组心率、房颤次数、左心室舒张末内径(LVEDD)、左心房内径(LAd)、左心房最大容积(LAVmax)、P波最大时限(Pmax)、P波离散度(Pd)、A峰峰值流速(VA),治疗前差异无统计学意义(P＞0.05),治疗后,观察组与治疗前差异有统计学意义(P＜0.05),对照组除左心房内径(LAd)之外与治疗之前差异均有统计学意义(P＜0.05).观察组不良反应3例,不良反应发生率为5.45％,对照组不良反应15例,不良反应发生率为28.30％,差异有统计学意义(P＜0.05).结论 使用稳心颗粒联合胺碘酮和培哚普利治疗老年阵发性心房颤动的效果较为明确并且优于一般的临床治疗效果,安全性高,值得进一步研究分析和应用.%Objective To investigate the therapeutic effects and safety of Wenxin granules combined with amiodarone and perindopril on senile paroxysmal atrial fibrillation.Methods One hundred and sixteen elderly patients with paroxysmal atrial fibrillation who were treated in department of cardiology of our hospital from September 2014 and January 2016 were randomly divided into control group and observation group,with 58 patients in each group.The patients in control group were treated by amiodarone combined with perindopril,however,the patients in observation group,on the basis of control group,were treated by Wenxin granules.After treatment the therapeutic effects,safety and adverse reactions were observed and compared between two groups.Results The effective rate in observation group was 90.57％,which in control group was 67.27％,there was a significant difference between two groups (P ＜ 0.05).Before treatment there were no significant differences in ventricular rate,frequency of atrial fibrillation,left ventricular end-diastolic diameter (LVEDD),left atrial diameter (LAd),left atrial maximum volume (LAVmax),P wave maximum time limit (Pmax),P wave dispersion degree (Pd),A peak value flow velocity (VA) (P ＞ 0.05).There were significant differences in these indexes mentioned above in observation group before treatment and after treatment,however,in control group,except for LAd,there were significant differences in these indexes before treatment and after treatment,moreover,there were significant differences in these indexes after treatment between two groups (P ＜ 0.05).There were 3 patients with adverse reactions in observation group,with the incidence rate of adverse reactions being 5.66％,however,there were 15 patients with adverse reactions in control group,with the incidence rate of adverse reactions being 27.27％,there was a significant difference between two groups (P ＜ 0.05).Conclusion Therapeutic effects of Wenxin granules combined with amiodarone and perindopril on senile paroxysmal atrial fibrillation are obvious and superior to those of routine therapy,with better safety,therefor,which is worth researching and using further.
