HLA-B27在诊断早期强直性脊椎炎中的应用价值

Objective To explore the value of HLA-B27 detection for the early diagnosis of ankylosing spon-dylitis (AS). Methods HLA-B27 detection was conducted by flow cytometry (FCM) in 48 patients of AS (AS group), 78 patients with suspected AS (suspected AS group), 35 patients with rheumatoid arthritis (RA group), and 20 healthy individuals (the control group).The results were compared with X ray findings. Results The positive rate was significantly higher in AS group and suspected AS group than RA group and the control group (x2 = 87.6, P<0.01; X2 = 63.0, P<0.01). The positive rate in AS group was significantly higher that in suspected AS group (x2 = 10.3, P<0.01). The positive rate of HLA-B27 detection showed no statistically significant difference with that of X ray in AS group (x2 = 3.07,P>0.05), while the positive rate of HLA-B27 detection is significantly higher than that of X ray in suspected AS group (P<0.01). Conclusion The detection of HLA-B27 plays important roles in the early diagnosis of patients with AS.%目的 探讨HLA-B27检测在诊断早期强直性脊椎炎(AS)中的临床应用价值.方法 采用流式细胞仪(FCM)对48例确诊AS患者、78例可疑AS患者、35例风湿性关节炎患者和20例正常体检者进行HLA-B27抗原检测,并将前两组结果与X线照片结果比较分析.结果 AS组和可疑AS组的阳性率均明显高于非AS组(风湿性患者组和正常对照组)(x2=87.6,P＜0.01；x2=63.0,P＜0.01),并且AS组明显高于可疑AS组(x2=10.3,P＜0.01)；HLA-B27检测与X光照片在AS组时差异无统计学意义(x2=3.07,P＞0.05),但可疑AS组HLA-B27的阳性率明显高于X光照片(P＜0.0l).结论 在AS的早期诊断中,检测HLA-B27有着重要的价值.
