蝶骨嵴脑膜瘤31例显微外科治疗分析

摘要

Objective To investigate the surgical approach, removal method, surgical technique, complication and outcome of sphenoid ridge meningioma operated by microsurgery. Methods The clinical data of 31 cases with sphenoid ridge meningiomas operated microsurgery were analyzed retrospectively. Results Among the 31 cases, 23 (74.2%) were of grade I ?II of Simpson system, and 4 (25.8%) were of grade III-IV. The incidence of complications was 19.4%, with no mortality. Conclusion Appropriate approach should be determined based on localization, size, and perifocal structure (vessels and nerves) of tumor. Familiarity with microsurgical anatomy and skillful technique can increase total resection rate and reduce complication of sphenoid ridge meningioma.%目的 探讨蝶骨嵴脑膜瘤的显微外科手术入路、切除方法、手术要点、并发症的预防及手术效果.方法 回顾分析31例采用显微外科治疗的蝶骨嵴脑膜患者的临床资料.结果 按照Simpson切除标准,全切23例(Simpson Ⅰ～Ⅱ级)占74.2％,8例大部分切除(SimpsonⅢ～Ⅳ级)占25.8％.并发症发生率为19.4％,无死亡病例.结论 根据肿瘤的具体位置、大小以及与周围重要神经组织的关系选择正确的手术入路,熟悉术区显微解剖和熟练的显微外科技术是提高蝶骨嵴脑膜瘤全切率和减少并发症的重要因素.
