尤瑞克林对急性脑梗死患者不同时期CRP水平的影响

             

摘要

Objective To explore the curative effect of Urinary Kallidinogenase on the changes of serum C-reactive protein (CRP) in patients with acute cerebral infarction. Methods Eighty-six patients with acute cerebral infarction were randomly divided into treatment group (treated with aspirin+physiological saline+Xueshuantong injec-tion+Urinary Kallidinogenase for 14 d) and control group (treated with aspirin+physiological saline+Xueshuantong in-jection for 14 d). The changes of neural function and serum level of CRP were compared between the two groups at 0 d, 3 d, 7 d and 14 d respectively. Results The effective rate of treatment group (81.39%) treated for 14 d was higher than that of control group (69.77%) with statistically significant (P<0.05). Serum levels of C-reactive protein (CRP) and NIHSS of the two groups treated for 3 d, 7 d and 14 d were reduced than before, and the treatment group was signifi-cantly lower than control group (P<0.001). The levels of CRP and NIHSS decreased in the treatment group from day 3 after the treatment, which was significantly faster than those in the control group (P<0.001). Conclusion Urinary kal-lidinogenase could significantly alleviate inflammatory reaction in patients with acute cerebral infarction, accordingly, it could reduce degree of cerebral infarction and tissue damage. Urinary kallidinogenase could be a protective factor through improving neural function.%目的:观察尤瑞克林(人尿激肽原酶)对急性脑梗死患者治疗后不同阶段血清C反应蛋白(CRP)的影响。方法将86例脑梗死患者采用随机数字表法分为治疗组和对照组各43例,两组基础用药均为口服肠溶阿司匹林200 mg,1次/d,生理盐水250 ml加入血栓通0.45 g静脉滴注,1次/d。其中治疗组除上述基础用药外另用生理盐水250 ml加入尤瑞克林0.15 PNA,静脉滴注,1次/d,疗程14 d,比较两组疗效,并动态观察两组患者治疗前及治疗后3 d、7 d、14 d血清CRP水平、神经功能缺损评分(NIHSS)的变化。结果治疗组治疗14 d后的总有效率(81.39%)明显高于对照组(69.77%),其差异有统计学意义(P<0.05)。治疗组患者血清CRP水平、NIHSS评分在治疗后3 d、7 d、14 d均较治疗前降低,且治疗组血清CRP水平、NIHSS评分下降程度明显优于对照组(P<0.001)。治疗组治疗后第3天就开始出现血清CRP水平、NIHSS评分降低,明显早于对照组(P<0.001)。结论尤瑞克林能够减轻急性脑梗死患者体内炎性反应程度,从而减轻脑梗死的严重程度及组织损伤程度,可作为改善急性脑梗死患者的神经功能缺损的保护性因素。
