瓜氨酸、肠型脂肪酸结合蛋白、肠三叶因子联合检测对儿童危重症急性胃肠损伤的诊断价值

         

摘要

Objective To discuss diagnosis value of combined detection of citrulline, intestinal fatty acid bind-ing protein (IFABP), intestinal trefoil factor (ITF) for critically ill children with acute gastrointestinal injury (AGI). Methods A total of 100 cases of critical illness children in Department of Pediatrics in our hospital for treatment were included in the study from September 2014 to August 2015. According to whether the patients had AGI or not, the pa-tients were divided into observation group (AGI, n=52) and control group (without AGI, n=48). The levels of citrulline, IFABP, ITF, C-reactive protein (CRP) and calcitonin gene-related peptide (PCT) were compared between the two groups, and correlations between the levels of citrulline, IFABP, ITF and CRP, PCT were analyzed. Results The levels of citrul-line in the observation group were significantly lower than those in the control group [(15.08 ± 8.84)μmol/L vs (25.07 ± 12.05) μmol/L], and the levels of IFABP, ITF, CRP and PCT in the observation group were significantly higher than those in the control group [(1 373.38±410.55) ng/L vs (938.89±503.84) ng/L, (14.51±4.90) ng/mL vs (9.79±4.82) ng/mL, (60.65±21.08) mg/L vs (28.12±10.13) mg/L, (22.93±10.19) ng/mL vs (5.28±3.08) ng/mL]. The differences were statisti-cally significant (P<0.01). Diagnostic value of combined detection of citrulline, IFABP and ITF for critically ill children with AGI were the highest, with the sensitivity of 98.08%, specificity of 95.83%and the accuracy of 97.00%. Pearson correlation analysis showed that citrulline was negatively correlated with CRP and PCT (r=-0.661,-0.709, P<0.05). There was no significant correlation between ITF, IFABP and CRP and PCT (r=0.108, 0.201, 0.097, 0.113, P>0.05). Conclusion Combined detection of citrulline, iFABP and ITF has good application value in critically ill children with AGI and is worthy to be popularized in clinical diagnosis.%目的:探讨瓜氨酸、肠型脂肪酸结合蛋白(IFABP)、肠三叶因子(ITF)联合检测对儿童危重症急性胃肠损伤(AGI)的诊断价值。方法选择2014年9月至2015年8月在我院儿科住院治疗的危重症患儿100例,按患儿是否合并AGI分为观察组(合并AGI)52例和对照组(未合并AGI)48例。比较两组患者的瓜氨酸、IFABP、ITF、C反应蛋白(CRP)及降钙素原(PCT)水平;并分析瓜氨酸、IFABP、ITF与CRP及PCT水平的相关性。结果观察组患者的瓜氨酸水平显著低于对照组[(15.08±8.84)μmol/L vs (25.07±12.05)μmol/L],而IFABP [(1373.38±410.55) ng/L vs (938.89±503.84) ng/L]、ITF [(14.51±4.90) ng/mL vs (9.79±4.82) ng/mL]、CRP [(60.65±21.08) mg/L vs (28.12±10.13) mg/L]及PCT[(22.93±10.19) ng/mL vs(5.28±3.08) ng/mL]均显著高于对照组,差异均有显著统计学意义(P<0.01);瓜氨酸、IFABP及ITF三者联合检测对儿童危重症AGI的诊断价值最高,其中敏感度为98.08%,特异度为95.83%,准确度为97.00%。根据Pearson法分析显示,瓜氨酸与CRP及PCT均呈负相关(r=-0.661,-0.709;均P<0.05),而IFABP和ITF与CRP及PCT无明显相关性(r=0.108,0.201,0.097,0.113;均P>0.05)。结论瓜氨酸与IFABP和ITF三者联合检测对危重症AGI患儿的诊断具有较高价值,值得在临床诊断过程中推广应用。
