目的 调查不同程度抑郁成人癫痫患者的自杀风险,分析成人癫痫合并抑郁症患者自杀的危险因素.方法 采用自评抑郁量表(SDS)及简明国际神经精神访谈量表(M.I.N.I.),对256例成人癫痫患者的抑郁及自杀风险进行评估.结果 256例成人癫痫患者中无抑郁者103例(40.2%),轻度抑郁者78例(30.5%),中度抑郁者53例(20.7%),重度抑郁者22例(8.6%).重度抑郁组患者无自杀风险患者比例(9.1%)明显低于无抑郁组(50.5%)、轻度抑郁组(51.3%)及中度抑郁组(37.7%),差异均有统计学意义(P<0.05);重度抑郁组患者自杀高度风险患者比例(31.8%)明显高于无抑郁组(6.8%)、轻度抑郁组(6.4%)及中度抑郁组(11.3%),差异均有统计学意义(P<0.05);Logistic回归模型分析显示,重度抑郁、发作频率、失业、多种药物治疗是自杀高度风险的危险因素.结论 成人癫痫合并重度抑郁患者的自杀风险高,应积极对癫痫患者抑郁状况进行评估,早期识别及处理高自杀风险的患者.%Objective To investigate the risk of suicide in adult epileptic patients with depression. Methods A total of 256 adult patients with epilepsy from the Second People's Hospital of Foshan were en-rolled to evaluate the risk of depression and suicide by Self-Rating Depressing Scale (SDS) and the MINI-Internation-al Neuropsychiatric Interview (M.I.N.I). Results Among the 256 patients, there were 103 patients without depression (40.2%), 78 patients with mild depression (30.5%), 53 patients with moderate depression (20.7%), and 22 patients with severe depression (8.6%). The proportion of no suicide risk in patients with severe depression (9.1%) was significantly lower than that in the patients without depression (50.5%), the patients with mild depression (51.3%) and the patients with moderate depression group (37.7%). The differences were all statistically significant (P<0.05). The proportion of high suicide risk in patients with severe depression (31.8%) was significantly higher than that in the patients without de-pression (6.8%), the patients with mild depression (6.4%), and the patients with moderate depression (11.3%). The differ-ences were all statistically significant (P<0.05). Logistic regression analysis showed that severe depression, frequency seizure, unemployment and multiple drug treatment were the risk factors of high suicide. Conclusion Adult epileptic patients with severe depression had high risk of suicide. Assessment of depression in patients with epilepsy is important to the recognition and treatment of patients with high risk of suicide.
