Objective To investigate the value of serum procalcitonin (PCT) in the diagnosis of ventilator-asso-ciated pneumonia (VAP) in patients treated with mechanical ventilation. Methods Thirty VAP patients in the ICU of our hospital from January 2014 to March 2015 were included into the VAP group, and 30 patients receiving mechanical ventilation and without VAP were included into the non-VAP group. The serum PCT and simplified clinical pulmonary infection score (sCPIS) were compared before and after mechanical ventilation. The VAP patients were divided into the im-proved group and deteriorated group according to the treatment outcome, and serum PCT level was compared between the two groups. Results The serum PCT and sCPIS level, and PCT positive rate in the VAP group were significantly in-creased after mechanical ventilation, while the indexes showed no statistically significant change in the non-VAP group (P>0.05). The indexes after mechanical ventilation were significantly higher in the VAP group than the non-VAP group (P<0.05). The serum PCT level in the improved group was significantly decreased compared with before treatment, and the level was increased significantly in the deteriorated group (P<0.05). Conclusion The increase of serum PCT level and CPIS is of great value in the diagnosis of VAP, and PCT change can determine the prognosis of VAP.%目的 探讨血清降钙素原(PCT)在诊断机械通气患者呼吸机相关性肺炎(VAP)中的应用价值.方法 选择2014年1月至2015年3月我院重症监护病房收治的30例临床确诊VAP患者纳入VAP组,同时选择30例同期接受机械通气治疗、未合并VAP患者纳入非VAP组,比较两组患者机械通气治疗前后的血清PCT及简化临床肺部感染评分(CPIS)水平;根据VAP患者疗效将其分为病情好转组及恶化组,比较两组患者血清PCT水平的变化.结果 VAP组患者血清PCT水平及简化CPIS评分在机械通气治疗后较治疗前明显升高,并且血清PCT阳性检出率明显增高,且上述指标均高于非VAP组水平,差异均有统计学意义(P<0.05),非VAP组上述指标无明显变化(P>0.05);VAP病情好转组患者血清PCT水平较治疗前明显下降,而病情恶化组患者明显上升,差异均有统计学意义(P<0.05).结论 血清PCT水平及简化CPIS评分升高对诊断VAP具有重要价值,并且监测血清PCT的变化可用于VAP的预后判断.
