30 mL、80 mL及150 mL小水囊引产用于足月妊娠促宫颈成熟疗效比较

         

摘要

目的 研究30 mL、80 mL及150 mL三种容量小水囊引产对足月妊娠促宫颈成熟所产生的临床效果.方法 选取2014年10月至2016年10月间于南京市江宁医院行引产治疗的宫颈条件不成熟足月妊娠患者177例为观察对象.采用随机数表法将其分为30 mL组、80 mL及150 mL组,各59例,分别接受30 mL、80 mL及150 mL无菌NaCl溶液的小水囊引产治疗.研究各组患者促宫颈成熟效果评估、宫颈评分、分娩情况及并发症发生情况,以评估上述三种方案促宫颈成熟的临床效果.结果 30 mL组、80 mL组及150 mL组患者促宫颈成熟效果评估中,有效患者人数所占组别总人数比重分别为84.75%、96.61%和98.31%,80 mL及150 mL组与30 mL组比较差异有统计学意义(P<0.05);80 mL及150 mL组患者引产后宫颈评分远高于30 mL组,且差异具有统计学意义(P<0.05);经阴道分娩患者人数所占比重、引产至临产时间和第一产程时间,80 mL和150 mL组患者表现情况均明显优于30 mL组,且差异具有统计学意义(P<0.05);三组患者在新生儿体质量和新生儿窒息率方面比较差异均无统计学意义(P>0.05);150 mL组患者脐带脱垂率远高于30 mL组和80 mL组,差异有统计学意义(P<0.05),而30 mL组和80 mL组比较差异无统计学意义(P>0.05);三组患者中出现并发症人数所占组别总人数比重差异无统计学意义(P>0.05).结论 80 mL小水囊引产治疗方案的综合治疗效果比30 mL和150 mL更突出,且副作用更小.%Objective To explore the clinics effect of ballon with 30 mL, 80 mL and 150 mL volume for cervi-cal ripening and induction of labor. Methods From October 2014 to October 2016, 177 premature pregnant women with cervical conditions treated by induction of labor in our hospital were selected and divided into 30 mL group, 80 mL group and 150 mL group, with 59 cases in each group. These three groups respectively accepted induced laber with small water bag of 30 mL, 80 mL and 150 mL NaCl sterile solution. The effect of cervical maturation, cervical score, delivery and complications of each group of patients were checked and used to evaluate the clinical effect of the above three pro-grams to promote cervical ripening. Results The proportion of the total number of effective patients in the 30 mL, 80 mL and 150 mL groups were respectively 84.75%, 96.61% and 98.31%. The differences of the 80 mL and the 150 mL group comparing with the 30 mL group were statistically significant (P<0.05). The cervical scores of the 80 mL and 150 mL group were significantly higher than that of 30 mL group (P<0.05). The incidences of vaginal delivery in the 80 mL and 150 mL patients were significantly higher than that in the 30 mL group (P<0.05). There was no significant difference in neonatal weight and neonatal asphyxia rate between the three groups (P>0.05). The umbilical cord prolapse rate in the 150 mL group was significantly higher than that in the 30 mL group and 80 mL group (P<0.05), but there was no signifi-cant difference between the 30 mL group and 80 mL group (P>0.05). There was no significant difference in the propor-tions of the total number of patients between the three groups (P>0.05). Conclusion The comprehensive treatment effect of 80 mL small water bag induction therapy was more prominent than 30 mL and 150 mL, which also has less side effects.
