Objective To explore influence on pancreatic beta cells and vascular endothelial cell function of treating newly diagnosed T 2DM patients with intensive insulin therapy in short-term . Methods 110 cases of newly diagnosed T2DM patients were selected and were randomly divided into group A and group B ,which were 55 cases for each group .Patients in group A were given oral hypo-glycemic drug ,and patients in group B were given intensive insulin treatment ,and they were all treat-ment for 2 weeks .Two groups'blood glucose (FBG ,2hPPG) ,HbA1c ,BG recovering time were test-ed ,Homa-βand Homa-IR were calculated ,serum NO and ET were observed ,and FMEDD and NIEID were measured by ultrasound before and after treatment .Results After treatment ,two groups'FBG , 2hPPG ,HbA1c ,Homa-IR and ET levels were decreased (P<0 .05or P< 0 .01) ,yet Homa-β and NO were increased (P<0 .05 or P<0 .01) .Group B's BG recovering time ,Homa IR and ET level were lower than that of group A (P<0 .05) ,and Homa-β,NO and FMEDD level were higher than that of group A (P<0 .01) .Conclusion It effectively controls blood glucose ,and protects the function of pancreatic beta cells and vascular endothelial cells of treating newly diagnosed T 2DM patients by in-sulin intensive therapy in short-term ,and it has positive role in controlling T2DM's progression and complications .%目的：探讨短期胰岛素强化治疗对初诊2型糖尿病（T2DM ）患者胰岛β细胞及血管内皮细胞功能的影响。方法选取初诊T2DM 患者110例，随机分为A、B组，每组各55例。A组给予口服降糖药治疗，B组给予短期胰岛素强化治疗，均治疗2周。于治疗前后比较两组患者的血糖（FBG、2 h PPG）、HbA1c和血糖达标时间，胰腺β细胞功能指数（Homaβ）和胰岛素抵抗指数（Homa IR），血清一氧化氮（NO）和内皮素（ET）水平，超声仪测定计算内皮依赖性血管舒张功能（FMEDD）、非内皮依赖性舒张功能（NIEID）。结果治疗后，两组FBG、2 h PPG、HbA1c、Homa IR、ET水平均降低（P＜0．05或 P＜0．01），Homa-β、NO升高（P＜0．05或 P＜0．01）；B组血糖达标时间、Homa IR、ET少于A组（P＜0．05），Homa-β、NO、FMEDD高于A组（P＜0．01）。结论短期胰岛素强化治疗对初诊2型糖尿病具有良好的临床疗效，不仅有效控制了血糖水平，也保护了胰腺β细胞及血管内皮细胞功能，对控制T2DM 进展和并发症的发生具有积极作用。
