目的：比较气管内滴入与静脉注射盐酸氨溴索注射液治疗急性呼吸窘迫综合征的疗效。方法：选取新乡市中心医院收治的急性呼吸窘迫综合征患者90例作为研究对象。将所有患者以随机数字表法分为观察组和对照组各45例。观察组患者给予气管内滴入盐酸氨溴索注射液，对照组患者给予静脉注射盐酸氨溴索注射液。比较2组患者的肺泡氧合功能、肺顺应性以及炎症介质水平。结果：观察组患者血氧分压、血氧饱和度、肺泡氧合指数均明显高于对照组，差异均有统计学意义（ P＜0.05）；观察组患者气道阻力明显低于对照组，肺顺应性明显高于对照组，差异均有统计学意义（ P＜0.05）；观察组患者白细胞介素1、白细胞介素6、肿瘤坏死因子α水平均明显低于对照组，差异均有统计学意义（ P＜0.05）。结论：采用气管内滴入氨溴索治疗急性呼吸窘迫综合征，可改善肺泡氧合功能，提高肺顺应性，缓解炎症反应。%OBJECTIVE:Compared the efficacy of ambroxol hydrochloride injection intratracheal instillation with intravenous injection of ambroxol hydrochloride injection in treatment of acute respiratory distress syndrome . METHODS:90 patients with acute respiratory distress syndrome admitted into Xinxiang Central Hospital were selected to be divided into observation group and control group via the random number table , with 45 cases in each .The observation group were treated with intratracheal instillation , while the control group received intravenous injection of ambroxol hydrochloride injection .The alveoli oxygenation function , lung compliance and inflammatory mediators level were compared between two groups .RESULTS:The PaO2 , SaO2 , PaO2/FiO2 of observation group were significantly higher than those of control group , with statistically significant difference ( P<0.05 ) .However , the airway resistance of observation group was significantly lower than that of control group , the lung compliance was significantly higher than that of control group, with statistically significant difference (P<0.05).The level of serum IL-1, IL-6, TNF-αof observation group were significantly lower than those of the control group , with statistically significant difference ( P<0.05 ) . CONCLUSIONS: Ambroxol hydrochloride injection intratracheal instillation and intravenous injection of ambroxol hydrochloride injection in treatment of acute respiratory distress syndrome can improve the alveolar oxygenation function and lung compliance , and relief the inflammation .
