我院2015年上半年住院患者抗肿瘤药物不合理用药医嘱分析

摘要

Objective: To promote the rationality of Antitumor Drugs intravenously.Methods: Intravenous prescriptions for inpatients in our hospital the first half year of 2015 were checked manually according to drug package insert,《New Materia Medica》, 10560 intravenous medical orders were reviewed. Irrational medical orders which fed back and recorded were retrospectively analyzed.Results: The unreasonable medical orders which have been audited run up to 210. Through classified statistics and analysis, unreasonable medical orders was found in all departments of our hospital, the worst is general surgery(25.56%), the second is department of gynecology(20.12%); the irrational medical orders types perform mainly in solvent type(46.12%), solvent amount(32.72%), drug dosage(14.12%)and other aspects(7.04%).Conclusion: Through the regular audit of intravenous medication orders and analysis and summary, the problems of intravenous administration in the clinic could be attached to timely management. At the same time, clinical pharmacists increase the ability of auditing prescription and establish the specification for the rational drug use. The summary and intervention of unreasonable orders can reduce adverse drug reactions and even accidents of drug use, in order to guarantee the safety of drugs and patients.%目的：促进抗肿瘤药物静脉用药的合理性。方法：依据药物说明书、《新编药物学》等对我院2015年上半年住院医嘱进行人工审核，共审核10560份静脉用药医嘱，将反馈并记录的不合理用药医嘱进行了回顾性分析。结果：共审核出不合理医嘱210条，将不合理用药医嘱进行分类统计与分析。发现我院各科室均存在不合理用药情况，最为严重为普外科(25.56%)，其次为妇科(20.12%)；不合理医嘱类型主要表现在溶媒种类(46.12%)、溶媒用量(32.72%)、给药剂量(14.12%)以及其他方面(7.04%)。结论：临床药师通过定期审核静脉用药医嘱并经分析及总结，将抗肿瘤药物应用中存在的问题及时反馈给临床科室；同时提高审方能力，建立合理用药规范，从而避免或减少药品不良反应甚至用药事故的发生，确保抗肿瘤药物的合理使用以及患者的生命安全。
