血清C肽与糖化血红蛋白联合检测在糖尿病诊断中的临床价值

             

摘要

目的:探讨血清C肽与糖化血红蛋白联合(HbAlc)检测在糖尿病诊断中的临床价值。方法选取该院2012年3月-2013年8月60例糖尿病患者,其中糖尿病组为单纯2型糖尿病患者(40例),糖尿病肾病组为糖尿病肾病患者(20例),并选取同时期健康体检者15例为对照组,检测各组HbA1c、空腹血糖(FBG)、餐后2 h血糖、血清C肽等指标变化。结果糖尿病组、糖尿病肾病组患者血清HbA1c、FBG、餐后2 h血糖对比对照组均明显上升,血清C肽水平明显降低,差异有统计学意义(P<0.05);糖尿病组与糖尿病肾病组患者FBG、餐后2 h血糖水平对比差异无统计学意义(P>0.05);糖尿病肾病组HbA1C水平显著高于糖尿病组,血清C肽水平显著低于对照组(P<0.05)。HbAlc与FBG具有正相关性,与C肽未负相关性。结论血清C肽与糖化血红蛋白联合检测对于糖尿病患者具有重要诊断价值,值得临床应用推广。%Objective To investigate the clinical value of combined detection of serum C-peptide and glycosylated hemoglobin (HbAlc) in the diagnosis of diabetes mellitus. Methods Sixty cases with diabetes admitted in our hospital between March 2012 and August 2013 were selected and divided into the diabetes group(40 cases) and diabetic nephropathy group(20 cases). Patients in the diabetes group had type 2 diabetes, while those in the diabetic nephropathy group had diabetic nephropathy. And 15 cases under-went health examination during the same period were selected as the control group. And the level of HbA1c, fasting blood glucose (FBG), 2h postprandial blood glucose, serum C-peptide and other indexes of the groups were tested. Results Compared with the control group, the level of HbA1c, FBG, 2h postprandial blood glucose of the diabetes group and the diabetic nephropathy group was obviously higher, while that of the serum C-peptide was much lower, the differences were statistically significant (P<0.05);there were no statistically significant differences in the level of FBG and 2h postprandial blood glucose between the diabetes group and the diabetic nephropathy group (P>0.05); the level of HbA1C of the diabetic nephropathy group was much higher than that of the diabetes group, while that of serum C-peptide was significantly lower than that of the control group (P<0.05). HbAlc was positively correlated with FBG, but was negatively correlated with C-peptide. Conclusion The combined detection of serum C-peptide and glycosylated hemoglobin has an important value in the diagnosis of diabetic patients, therefore it is worthy of clinical application.
