Objective To discuss the factors influencing the outcome of patients with trigeminal neuralgia treated by microvascular decompression. Methods Patients with trigeminal neuralgia who received microcasular were review. Factors including disease duration, age of onset, symptom typicality, type of responsible vascular and their compressing degree to nerve roots were described and their relationship with operative outcome were analyzed. Results Patients with typical symptoms, duration less than 2 years and responsible vascular only contacting nerve roots had a higher ratio of complete and delayed release than those with untypical symptoms, longer duration and complicated relationship between responsible vascular and nerve roots. Conclusion Disease duration, symptom typicality, compression degree of responsible vascular to nerve roots are the independent factors that can influence the operative outcome of microvasclur decompression to patients with trigeminal neuralgia.%目的：探讨影响微血管减压术治疗三叉神经痛疗效的因素。方法选择68例原发性三叉神经痛患者进行微血管减压术治疗，详细描述病程、发病年龄、症状典型性、责任血管类型、压迫程度等因素，分析这些因素与手术疗效的关系。结果病程小于2年、症状典型、责任血管仅与神经相粘连者术后完全缓解、延迟缓解比例较高，相反，病程长、症状不典型、责任血管与神经关系复杂者疗效较差，差异有统计学意义。结论病程、症状是否典型、血管对三叉神经根的压迫程度是影响微血管减压术治疗三叉神经痛疗效的独立因素。
