电脑膀胱镜模拟器在住院医师膀胱镜检查培训中的教学作用

         

摘要

目的 了解使用电脑膀胱镜模拟器对住院医师膀胱镜检查培训的效果.方法 选择2015年1-2017年3月在某院外科培训基地接受标准化培训的20名住院医生,随机分为两组,A组为传统教学组,B组为模拟器教学组.两组均接受理论授课后,两组均直接进入膀胱镜室见习3台真实膀胱镜检查,B组见习后,先接受为期1天的URO mentor膀胱镜模拟器标准化课程训练.之后两组住院医师均在上级指导下进行6台膀胱镜检查操作,由上级医生按最后两次操作的表现打分,相关数据录入自建数据库.结果 参加研究两组住院医生进入规范化培训时间A组平均17.5±3.0月,B组15.2±2.8月,两组受试者教育水平、既往其他内镜操作经验等方面均无显著性差异.考核评分B组优于A组(85.1±1.2 vs. 88.9±1.1,P<0.05),且B组住院医师在实际操作中自信度、熟练度等方面优于A组.结论 URO Mentor模拟操作可以有效的提高住院医师掌握膀胱镜操作的进度,值得在住院医师规范化培训中进行推广.%Objectives To assess the effect of applying the computer based cystoscopy simulator (URO Mentor) for the resident training program. Methods 20 residents who received standardized surgical training at Beijing Friendship Hospital during Jan 2015 to Mar 2017 were involved to present study. All the residents were randomly divided into two groups: group A was a traditional teaching group and group B was simulator group. After theoretical lectures, both two groups entered the cystoscopy room and looked six real cystoscopy as internship. After internship, Group B received 1 days of URO mentor cystoscopy simulator standardized course training. Then, two groups were conducted 6 cystoscopy under the guidance of their superiors. Teacher scored according to their last two performance. Results The resident training time was 17.5 + 3 months (group A) and 15.2 + 2.8 months (group B). The assessment score of B group was superior to group A (85.1 + 1.2 vs. 88.9 + 1.1, P<0.05), and the confidence degree and proficiency of the residents in group B were better than group A. Conclusions Computer-based endourologic simulation could effectively improve the learning progress of residents in cystoscopy training and is worthy of promotion in surgical residency standardized training.
