超声心动图评估室间隔完整合并体-肺侧支形成肺动脉闭锁

摘要

Objective To explore the value of echocardiography in the diagnosis of pulmonary atresia combined with intact ventricular septum(PA/IVS).Methods 24 cases of PA/IVS patients were diagnosed by echocardiography,focusing on the subxyphoid,suprasternal and left high precardial views.Results In 24 cases of PA/IVS,23 cases were diagnosed correctly by echocardiography and 1 case was misdiagnosed.There were 6 cases of pulmonary valve and funnel hypertrophy atresia,17 cases of membranous atresia of pulmonary artery.And the pulmonary artery was fascicular in 1 case of PA/IVS.All patients had intact ventricular septum.,shunt from right atrial to left atrial and patent ductus arteriosus.Conclusions Echocardiography has a high diagnostic value for PA/IVS and a great significance in the clinical evaluation.It is helpful to guide the surgery and estimate the prognosis.%目的 探讨超声心动图评估肺动脉闭锁伴室间隔完整(PA/IVS)合并体-肺侧支的价值.方法 应用超声心动图诊断PA/IVS 24例,重点采用剑突下、胸骨上窝、胸骨旁高位切面进行检查.结果 24例PA/IVS中超声心动图诊断正确23例,误诊1例;肺动脉瓣及漏斗部肥厚闭锁6例,肺动脉瓣呈膜性闭锁17例,肺动脉主干呈条束状1例;所有患儿室间隔均连续完整,均有心房水平的右向左分流,均合并动脉导管未闭.结论 超声心动图对PA/IVS具有较高的诊断价值,对该类疾病的临床评估具有重要意义,有助于指导外科手术并评估预后.
