超声极速成像技术评价类风湿关节炎患者颈动脉脉搏波传导速度应用价值的临床研究

         

摘要

目的 探讨运用超声极速成像技术(UF)评价类风湿关节炎(RA)患者颈动脉脉搏波传导速度(PWV)的应用价值.方法 依据2010年ACR/EULAR提出的RA分类标准,纳入62例RA患者作为RA组,并根据关节X线分期分为非骨侵蚀组及骨侵蚀组,另招募42例健康志愿者作为对照组.收集临床资料,且均行颈超扫查,测量颈动脉平均内中膜厚度(IMT),并运用UF测量颈动脉收缩早期及收缩晚期脉搏波传导速度(BS和ES).结果 RA组BS、ES及IMT较对照组显著增高(P<0.05);RA患者骨侵蚀组BS、ES显著高于非骨侵蚀组(P<0.05),而两组间IMT无明显统计学意义(P>0.05).BS、ES与IMT呈正相关,相关系数r分别为0.32、0.25.结论 UF可为检测RA患者早期动脉粥样硬化提供参考依据.随着病情进展,RA患者动脉弹性下降.%Objective To investigate the ultrafast ultrasound (UF)for evaluation of carotid pulse wave velocity (PWV) in patients with RA.Methods Based on the 2010 standard in RA of ACR/EULAR,enrolled 62 cases of RA patients as RA group and divided into two subgroups with or without bone erosion.Recruited another 42 cases of healthy volunteers as control group.Collected the clinical data,and underwent ultrasound measurement of carotid artery IMT,detected early and late systolic PWV (BS and ES).Results The BS,ES and IMT in RA group were significantly higher than that of control group and significantly different(P<0.05),The BS,ES in bone erosion group were significantly higher than that in group of without bone erosion (P<0.05),however IMT had no significant difference (P>0.05).The BS,ES were positively associated with IMT,the r was 0.32,0.25.Conclusions UF can provide basis for early detection of atherosclerosis for RA patients,the disease progressed,the arterial elasticity decreased.
