彩色多普勒超声检测系统性红斑狼疮亚临床心脏异常

摘要

应用彩色多普勒综合性心脏超声方法(彩超)检测30例初发系统性红斑狼疮(SLE)连续病例并与30例健康对照组比较.结果显示SLE组左室内径及室壁厚度较大;心室肥厚和/或扩张占33%;左室充盈指标异常占40%;瓣叶病变、心包积液和肺动脉高压分别占43%、37%和27%.总异常率为87%,远高于常规心电图及x线胸片(P<0.01).认为SLE年青患者病程早掰即多有亚临床心脏异常,彩超是其早期诊断及随访观察的简使可靠的无创性检查方法.%A color Doppler echocardiography(CD Echo)was performed in 30 new onset patients with systemic lupus erythematosus(SLE)and 30 normal controls.CD Echo abnormalities were more common(26 cases,87%)than that of the electrocardiogram and the chest film in SLE(P<0.01;P<0.01).Valve abnormalities,pericardial effusion and pulmonary hypertension were found in 13,11 and 8 patients(43%,37%,and27%),respectively.Abnormalities in ventricular chamber size and/or wall thickness and left ventricular filling were found in 10 and 13 patients(33% and 40%),respectively.These data indicate that there are a variety of subclinical cardiac abnormalities in SLE and Confirm the value of CD Echo for identifying and monitoring these abnormalities.
