剪切波弹性超声评价神经慢性卡压的弹性变化

             

摘要

目的:利用剪切波弹性技术评价神经慢性卡压性病变的组织硬度变化.方法:10只新西兰大白兔建立坐骨神经慢性卡压的动物模型,于术前和术后2、4、6、8周分别对坐骨神经进行二维图像观察和剪切波弹性测量.结果:随时间的延长,卡压两端神经直径逐渐增粗,卡压两端神经的杨氏模量值也逐渐增大.术前正常神经的杨氏模量值为(6.53±2.02)kPa,卡压8周时近端神经为(17.77±4.09)kPa,远端神经为(15.19±4.46)kPa,卡压后各时间点与术前神经的直径和杨氏模量均具有统计学差异(P<0.05).增粗的神经直径与杨氏模量呈正相关(r=0.78,P<0.05).结论:神经慢性卡压后,肿胀增粗的神经较正常神经变硬.剪切波弹性成像能有效地反映神经卡压后的弹性硬度变化,为神经卡压病变提供一种新的诊断依据.%Objective To evaluate the changes of the tissue stiffness of chronic entrapment disease using shear wave elastography. Methods Animal model of chronic sciatic nerve entrapment were established, the sciatic nerve was observed by two dimensional images and measured by shear wave elasticity at pre-operative and post-operative 2,4,6 and 8 weeks. Results With the extension of time, the diameter of the nerve at both ends of the entrapment gradually increased. The Young's modulus of the pre-operative normal nerve was (6.53± 2.02) kPa, and entrapment at 8 weeks, the proximal nerve was (17.77±4.09) kPa and the distal nerve was (15.19± 4.46) kPa, the diameter and Young's modulus of the nerve at each time point after entrapment and the pre-oper-ation were statistically significant (P<0.05). The increase of the nerve diameter and Young's modulus were pos-itively correlated (r=0.78,P<0.05). Conclusion The swelling and thickening nerve after the chronic nerve entrapment is harder than the normal nerve. Shear wave elastography can effectively reflect the change of the elastic stiffness of the nerve after the entrapment, which can pave the way for the diagnosis of nerve entrapment.
