目的 探讨武汉市低出生体重儿发生的危险因素.方法 对武汉市妇幼保健信息管理系统中2011—2015年的508333例新生儿的出生体重及其影响因素进行调查分析.结果 武汉市低出生体重儿的发生率由2011年的4.92％ 上升至2015年的5.34％,呈明显的上升趋势(χ2=25.7231,P<0.0001),5年的平均发生率为5.10％;多因素分析结果显示,影响低出生体重儿发生的主要危险因素有:孕产妇低龄[<20岁,OR:1.21(1.01,1.44)]或高龄[≥35岁,OR:1.98(1.71,2.29)]、高危[OR:1.98(1.71,2.29)]、早产[(OR:27.22(14.20,52.18)]、多胎[(OR:8.09(4.66,14.07)]、女婴[OR:1.46(1.15,1.85)]及孕前BMI<18.5[(OR:1.12(1.04,1.20)]或BMI≥24.0[OR:1.13(1.05,1.22)]、BMI≥28[OR:1.14(1.03,1.26)]等.结论 武汉市低出生体重儿的发生率较高并呈明显的上升趋势,值得高度重视;影响武汉市低出生体重儿发生的危险因素是多方面的,应通过健康宣教,尽量避免或降低低龄或高龄孕产,控制高危、早产、多胎及孕前BM I等综合措施来降低武汉市低出生体重儿的发生率.%Objectives To explore risk factors of the low birth weight infant (LBWI)incidence in Wuhan.Methods The birth weight and its influencing factors of 508333 newborns were administered ,utilizing data from Wuhan maternal and children information management system between 2011 and 2015.Results LBWI incidence in Wuhan increased obviously from 4.92％ in 2011 to 5.34％ in 2015(χ2 =25.7231 ,P for trend<0.0001) ,with an average of five years incidence of 5.10％.Multivariate analysis showed that main risk factors of LBWI incidence were ,the ma-ternal age(less than 20 ,OR:1.21(1.01 ,1.44))or elder(35 and higher ,OR:1.98(1.71 ,2.29)) ,high risk during pregnancy(OR:1.98(1.71 ,2.29)) ,pre-term(OR:27.22(14.20 ,52.18)) ,multiple birth(OR:8.09(4.66 ,14.07)) , female infant(OR:1.46(1.15 ,1.85)) ,and pre-pregnancy BMI <18.5(OR:1.12(1.04 ,1.20)) ,BMI ≥24.0(OR:1.13(1.05 ,1.22)) ,BMI ≥28(OR:1.14(1.03 ,1.26)).Conclusions LBWI incidence was high and increased clearly with years in Wuhan which should be taken more attention.Since the risk factors of LBWI incidence are various ,we can reduce the incidence of LBWI by taking measures such as health education ,reducing the young or elderly preg-nancy ,controlling the high risk during pregnancy ,premature birth ,multiple pregnancy and pre-pregnancy BM I.
