神经根无菌性炎症与直腿抬高试验阳性相关影像学测量指标

         

摘要

Objective To analyze the imaging indicators relative to the nerve root of the patients with lumbar disc hernia-tion (L .D .H) and positive straight-leg raising test ,whose discs of L4/5 and L5/S1 are involved ,and to discuss the primary cause of lower lumbar pain and the pathological changes .Methods 58 LDH patients with positive straight-leg raising test and 30 nor-mal persons ware collected as the subject ,and lumber CT scanning were performed for all of them .First ,according to the loca-tion of lumbar disc herniation ,the groups were divided as the L4/5 group and L5/S1 group .Then ,each of them was divided into 3 groups according to the angle of the straight-leg raising test ,0°~30° ,30°~60°and >60° .CT value of the tissue around the never root(NR) ,CT value of the NR ,diameter of the NR were be measured .Analysis of variance and t-test were made for these data .Results Analysis of variance and t-test through each of the groups showed that there were significant differences in CT value of the tissue around NR ,CT value of the NR ,diameter of the NR among all groups (P<0 .01) .Conclusion Compression of NR ,edema and inflammation reaction have important relationship with the angle of the straight-leg raising test in LDH pa-tients .Application of imaging examination may provide an excellent assessment and stage of the inflammation .%目的:通过分析直腿抬高试验阳性的腰椎间盘突出症患者L4/5、L5/S1水平受累神经根相关影像学测量指标,探讨导致患者下腰痛的根本原因及其相关病理改变。方法收集我院近期直腿抬高试验阳性的腰椎间盘突出累及L4/5、L5/S1水平患者58例(患病组),正常志愿者30例(对照组),均行腰椎C T平扫检查。依据椎间盘突出病变位置分为L4/5、L5/S12组,再依据其直腿抬高试验阳性角度将以上2组分别分为强阳性组、阳性组和弱阳性组。分别测量各组L4/5、L5/S1水平受累侧神经根周围组织CT值、神经根CT值、神经根直径。对所得数据进行统计学处理,患病各组与正常对照组间进行方差分析及 t检验,患病各组间进行方差分析。结果患病组与对照组神经根周围组织CT 值、神经根CT 值、神经根直径测量结果比较差异有统计学意义(P<0.05)。结论腰椎间盘突出症直腿抬高试验阳性角度与神经根受压、水肿及其神经根周围炎性改变密切相关。临床诊疗过程中积极结合影像学检查,实施辨证分期,避免不适当的治疗方法导致神经根最终粘连,以达到影像学检查指导临床诊疗的目的。
