喉内镜联合等离子射频消融切除喉肿瘤17例

摘要

目的 探讨喉内镜联合等离子射频消融在切除早期喉肿瘤手术中的联合作用.方法 在喉内镜显像系统监视下,运用等离子刀头切除喉部肿瘤病变17例,其中3例中低分化鳞状细胞癌患者术后加放射治疗.结果 全部喉部肿瘤病变一次性切除,手术过程顺利,所有病例均未行气管切开术,麻醉清醒后即拔管,术后5～7d出院,术后3个月喉部创面愈合良好.本组除1例患者6个月后复发,其余患者随访中均未复发,1例复发患者再次行等离子喉显微手术切除,至末次随访局部未再复发.术后无1例患者出现呼吸困难、出血等严重并发症.结论 喉内镜联合等离子射频消融可以应用于早期喉肿瘤手术中,具有安全、微创、暴露清晰、出血少、术后并发症少、住院时间短等优点,疗效满意.%Objective To investigate the clinical effect of coblation combined with suspension laryngoscopy for laryngeal neoplasms. Methords Seventeen patients with laryngeal neoplasms were treated by coblation combined with suspension laryngoscopy. Three cases of poorly differentiated squamous-cell carcinoma were treated with radiotherapy after surgery. Results All of the laryngeal neoplasms were removed completely without severe complications and tracheotomy. Patients were discharged 5-7 days after operation and the laryngeal wound healing was completed in 3 months. Within a following-up from 12 to 48 months, one case got recurrence in 6 months postoperatively, and recovered after second coblation surgery combined with suspension laryngoscopy. Conclusion Coblation combined with suspension laryngoscopy for laryngeal neoplasms is an effective and safe technique, and has the advantages as minimally invasion and low incidence of complication.
