替加环素不良反应的文献计量分析

摘要

目的:探究替加环素所致不良反应(ADRs)的发生规律及临床特点,促进临床安全合理用药.方法:以“替加环素、“不良反应”等为关键字,在中华医院知识仓库(CNKD)期刊全文数据库、万方数据库、维普中文科技期刊数据库、中华医学会期刊自建数据库、中国知网(CNKI)检索1997-2016年有关替加环素不良反应的文献,并以“tigecycline”,“adverse drug reactions”等为检索词在Pubmed和Elsevier Clinicalkey进行上述检索,对其进行统计分析.结果:共检索到文献1 439篇,排除综述、同一病例的重复报道及其余交代不清的报道,共有符合标准的文献44篇(国外27篇,国内17篇),病例169例.替加环素所致ADRs及其临床表现复杂多样,其中以急性胰腺炎、凝血功能障碍为主,其他还包括血细胞减少、腹泻、皮疹、肝损伤、肾损伤等.结论:替加环素所引起的ADRs日益增多,可累及全身多个系统,应引起临床高度重视.%Objective:To explore the regularity and clinical characteristics of adverse drug reactions (ADRs) induced by tigecycline in order to providing references for safe and rational medication.Methods:Using "tigecycline" and "ADRs" as key words,we searched the China Hospital Knowledge Database (CHKD),Wanfang Database,Chinese VIP Journal Database,Journal Database of the Chinese Medical Association,China National Knowledge Internet (CNKI),Pubmed database and Elsevier Clinicalkey between 1997 and 2016,and summarized and statistically analyzed the ADRs induced by tigecycline.Results:A total of 1 439 articles were retrieved.Summaries,the repeated reports of the same cases,and unclear reports were excluded.There were 44 papers (27 in foreign countries and 17 in China) and 169 patients were enrolled.The ADRs and clinical manifestations induced by tigecycline were complex and various,mainly included acute pancreatitis and blood coagulation dysfunction as well as blood cell reduction,diarrhea,skin rash,liver damage,kidney damage,and others.Conclusion:Tigecycline-induced ADRs show a trend of continuous increasing,and it could be involved in general systems.ADRs monitoring of tigecycline should be strengthened to ensure safe and rational medication.
