早期渐进性运动训练对ICU患者肌力及生活质量的影响

摘要

Objective To study the effects of early progressive exercise training on muscle strength and quality of life in ICU patients.Methods A total of 80 ICU patients who were treated in the First Affiliated Hospital of Zhengzhou University from May 2014 to May 2016 were selected as the research subjects.All the subjects were randomly divided into observation group and control group,with 40 patients in each.The two groups received routine care.In addition,the observation group were given early progressive exercise training.Patient outcome of ICU treatment were compared between the two groups,by Medical Research Council Score(MRC-Score),Barthel Index Score,36-Item Short Form Healthy Survey(SF-36).Results The rate of adverse event of the observation group was 7.5％,and it was 20.0％in the control group.The difference was statistically significant(χ2=4.021,P<0.05).The muscle strength of the 39 patients in the observation group recovered toⅣ-Ⅴ level at discharge.It was 31 in the control group,and the difference was statistically significant(Z=-2.093,P < 0.05).The Barthel Index of the observation group at discharge was(60.25±6.18),and the score fo the general health dimension of SF-36 was(58.49±7.12),which were both higher than those of the control group,with statistical significance(t=4.400,4.036;P<0.05).Conclusions The early progressive exercise training can help the recovery of muscle strength and self-care ability in ICU patients,and reduce the incidence of adverse events,which are great significance to improve the quality of life.%目的 探讨早期渐进性运动训练对ICU患者肌力和生活质量的影响.方法 选取2014年5月—2016年5月郑州大学第一附属医院收治的ICU患者80例,随机分为观察组和对照组,各40例.两组均接受常规护理,观察组同时行早期渐进性运动训练,比较两组患者ICU治疗结果,采用肌力测定法(MRC-Score)、Barthel指数评分、简明健康调查量表(SF-36)评定患者肌力状况、生活自理能力和生活质量.结果 观察组不良事件发生率7.5％,对照组20.0％,差异有统计学意义(χ2=4.021,P<0.05).观察组39例患者出院时肌力均恢复到Ⅳ～Ⅴ级,对照组只有31例,差异有统计学意义(Z=-2.093,P<0.05).观察组患者出院时Barthel指数(60.25±6.18)分,SF-36问卷中总体健康维度(58.49±7.12)分,均高于对照组,差异有统计学意义(t值分别为4.400、4.036;P<0.05).结论 早期渐进性运动训练有助于ICU患者肌力和生活自理能力的恢复,降低不良事件发生率,对改善生活质量具有重要意义.
