高血压脑出血并发肺部感染相关因素分析

         

摘要

目的:研究高血压脑出血患者并发肺部感染的相关因素。方法回顾性分析158例高血压脑出血病例,根据是否并发肺部感染分为感染组和非感染组。用logistic回归方程得出对并发肺部感染有重要意义的相关因素,用受试者工作特征( receiver operating characteristic curve, ROC )曲线分析诊断效率。结果年龄、吸烟史、格拉斯哥昏迷( Glasgow coma score,GCS)评分、白细胞、白蛋白共5项因素进入logistic回归方程(P<0.01)。 ROC曲线示logistic回归方程的预测概率具有较高的诊断效率。结论年龄、吸烟史、GCS评分、白细胞、白蛋白是高血压脑出血并发肺部感染的重要相关因素。 logistic回归结合ROC曲线在多因素分析时具有较高的诊断效率,有助于早期干预和临床诊治。%Objective To explore the related factors of hypertensive intracerebral hemorrhage complicated with pul-monary infection. Method A retrospective study, 158 patients with hypertensive intracerebral hemorrhage enrolled in the study. The patients were divided into infection group and non-infection groupaccording to whether with pulmo-nary infection. The related factors with great significance to hypertensive intracerebral hemorrhage complicated with pulmonary infection was screened out by logistic regression equation. The diagnostic efficiency of logistic regression equation was analyzed by receiver operating characteristic ( ROC ) curve. Result Logistic regression equation showed that the valuable related factors of hypertensive intracerebral hemorrhage complicated with pulmonary infec-tion included age, history of smoking, Glasgow coma score, leukocyte and albumin(P<0. 01). ROC curve analysis showed the predicted probability of logistic regression equation having higher diagnostic efficiency. Conclusion Age, history of smoking, Glasgow coma score, leukocyte and albumin were the significative related factors of hyper-tensive intracerebral hemorrhage complicated with pulmonary infection. The combination of logistic regression and ROC curve had higher diagnostic efficiency on multivariate analysis, which could contribute to early intervention and clinical diagnosis and treatment.
