Objective To investigate the clinical features, treatment and prognosis of lower extremity deep venous thrombosis (DVT) in pregnant women. Method 35 cases of perinatal maternal DVT found in Ningbo Women and Children's Hospital from 2015-01-01 to 2016-12-31 were selected. The clinical data of 35 patients were analyzed retrospectively. The time, type, risk factors, clinical manifestation, auxiliary examination, treatment and prognosis were analyzed. Result Perinatal maternal DVT occurred in 24 cases (68.6%) in puerperium, 28 cases (80%) in left lower extremities, 26 cases(74.3%) in peripheral type. High risk factors: perinatal irregular birth examination, low age or advanced age, long-term bedridden, pregnancy complications or medical diseases, past DVT history or family history of thrombosis. Swelling of the affected limbs is the most common clinical manifestation. 34 cases (97.1%) were diagnosed by ultrasound and 1 case (2.9%) were confirmed by deep venous angiography of the lower extremities. Pulmonary embolism was found in only 1 case (2.9%) with systemic anticoagulant and local thrombolytic therapy. Conclusion DVT and secondary PE are serious threat to maternal and fetal health and life, for high-risk pregnant women should be alert to the possible DVT, preventive anticoagulant therapy, timely diagnosis and effective treatment can improve its prognosis.%目的 探讨围产期孕产妇下肢深静脉血栓形成(deep venous thrombosis,DVT)的临床表现、治疗方法及预后.方法 回顾性分析宁波市妇女儿童医院2015年1月1日至2016年12月31日收治的35例围产期DVT孕产妇的临床资料,内容包括发病时间、类型、高危因素、临床表现、辅助检查、治疗及预后等.结果 围产期DVT多发生于产褥期[24例(68.6%)],左下肢多见[28例(80.0%)],周围型多发[26例(74.3%)].围产期不规律产检、低龄或高龄、长期卧床、具有妊娠合并症或内科疾病和既往DVT病史或血栓家族史是围产期DVT的高危因素.患肢肿胀是最常见的临床表现.34例(97.1%)孕产妇经超声诊断,1例(2.9%)经下肢深静脉造影确诊.所有孕产妇均采用全身抗凝及局部溶栓治疗,仅1例(2.9%)发生肺栓塞.结论 DVT及继发的肺栓塞严重威胁孕产妇及胎儿的健康和生命,对于高危孕产妇应警惕DVT的可能,预防性抗凝治疗,及时诊断、有效治疗可改善其预后.
