关节置换术后深静脉血栓患者血清超敏C-反应蛋白及白细胞介素的相关性分析

Objective:To investigate expression of the characteristics and interrelationships of high-sensitivity C-reactive protein and interleukin-6 (IL-6), IL-8, IL-10 in in patients with deep vein thrombosis after arthroplasty. Methods:The serum IL-6, IL-8, IL-10 and hs-CRP of 76 cases of patients with joint replacement arthroplasty were detected in 1, 2, 4 days. Whether the patient had the DVT generated was determined by Doppler ultrasound. Results:Varying degrees of DVT formation found in 31 cases of joint replacement patients after Doppler ultrasound (DVT group), found no DVT formation in 45 patients (control group);DVT group after 1 d of proinflammatory cytokines in serum hs-CRP, IL-6, IL-8 levels were significantly higher than the control group (P<0.05), however the anti-inflammatory cytokine IL-10 was significantly lower (P<0.05). The expression of hs-CRP and IL-6, IL-8 decreased gradually, IL-10 increased in DVT group after 1 d, 2 d, 4 d of arthroplasty. Serum hs-CRP were positively correlated with IL-6, IL-8 (r=0.9267,0.8883, P<0.05), hs-CRP was negatively correlated with IL-10 (r=-0.9903, P<0.05). Conclusion:The expression of hs-CRP and IL-6, IL-8, IL-10 were interrelated, through the expression of these inflammatory cytokines can guide DVT clinical diagnosis, treatment and assessment of prognosis.%目的：探讨关节置换术后患者血清白介素-6(IL-6)、IL-8、IL-10及超敏C反应蛋白(hs-CRP)在深静脉血栓(DVT)发生的表达特点及相互关系。方法：检测76例关节置换患者置换前及置换后1、2、4 d的血清IL-6、IL-8、IL-10及hs-CRP,并以多普勒超声检测以确定患者是否有DVT生成。结果：76例关节置换患者在术后多普勒超声检查发现具有不同程度DVT者31例(DVT组),未发现DVT者45例(对照组)；DVT组术后1 d的血清中致炎因子hs-CRP、IL-6、IL-8水平较对照组均明显升高(P<0.05),抗炎因子IL-10的水平较对照组明显降低(P<0.05)；经抗血栓治疗后,DVT组术后1 d、2 d、4 d的血清hs-CRP与IL-6、IL-8表达水平均逐渐降低,IL-10表达逐渐升高,且血清hs-CRP与IL-6、IL-8表达水平均呈正相关(P<0.05),hs-CRP与IL-10表达水平呈负相关(P<0.05)。结论：hs-CRP与IL-6、IL-8、IL-10相互关联,通过这些炎症因子的表达可指导关节置换术后DVT的临床诊断、治疗以及评估预后。
