左室Tei指数对妊娠高血压综合征患者左心功能的评估#

             

摘要

目的::本研究旨在运用彩色多普勒超声心动图的左室Tei指数对妊娠高血压综合征(Hyperensive disrderscomp Licating pregnancy,简称妊高征症,HDP)患者左心功能进行评估。方法:选择2011年1月~2012年1月我院妇产科收治的HDP患者269例,其中妊娠高血压116例,轻度子痫前期88例,重度子痫前期65例,并选择与其孕龄相匹配的血压正常孕妇200例,为对照组,应用超声心动图检测她们的等容舒张时间(IRT),等容收缩时间(ICT)和左室射血时间(ET),计算Tei指数。结果:高血压组116例中,82例Tei指数略有增高,34例增高不明显,增高数约占B组总数的70%,轻度子痫前期组88例中76例Tei指数增高,12例增高不明显,12例均为经产妇,约占C组总数的86%。重度子痫前期组65例中Tei指数均增高,增高率为100%,且血压越高、临床表现越严重,Tei指数增高越明显。结论:HDP患者的Tei指数存在着明显异常变化,HDP患者病情越重,Tei指数增高越明显,说明左心功能降低越显著。%Objective:To study the Tei-index of left-ventricular of paients with gestational hypertension (Hyperensive disrderscomp Licating pregnancy,HDP),and to evaluate patient's left heart function.Methods:269 patients with HDP from January 2011-to January 2012,who were in Department of Obstetrics and Gynecologyl,including 116 cases of gestational hypertension,88 cases of mild preeclampsia,65 cases of severe preeclampsia,and we selected their gestational age which were matched normal blood pressure in 200 cases pregnant women for the control group.We used the echocardiography to detect their isovolumetric relaxation time (IRT),isovolumetric contraction time (ICT) and left ventricular ejection time (ET),and calculated the Tei index.Results:In 116 cases of hypertension group,82 cases’ Tei index was slightly higher,34 cases’Tei index increase was not obvious,the increased number of accounts was about 70%of the total number of group B;In 88 cases of mild preeclampsia group,76 cases’ Tei index increased,12 cases’increase was not obvious,12 cases were multipara,who were accounted for about 86%of the total number of group C.Tei index in 65 cases of severe preeclampsia group was increased,the increase was the rate of 100%.The higher blood pressure,the more obvious of Tei index.Conclusion:Tei index of HDP patients has changed significantly.The illness is more serious,then the Tei index will increase more obvious.And this means that the left ventricular function in patients will decreased more significantly.
