楔形野与野中野在三维适形治疗计划的剂量对比研究

     

摘要

目的:比较研究楔形过滤板射野(Wedge Filter)与野中野(FIF:Field-in-Field)在放疗时靶区及周围正常组织照射剂量的差异.方法:随机选择骨(腰椎)转移癌,脑癌,乳腺癌,食管癌,肺癌共24例,进行CT扫描,靶区和危及器官(OAR)的勾画,用三维治疗计划系统进行楔形野计划(Wedge Plan)和野中野计划(FIF Plan)设计,并进行结果对比分析.结果:FIF计划的剂量均匀性(HI)和适形度(CI)要优于Wedge计划,P值分别为0.001,0.007,同时FIF计划的跳数明显小于Wedge计划的跳数,其差异具有统计学意义(P=0.00),而PTV的靶区覆盖率(V95%)和正常组织照射量差异没有明显的统计学意义(P>0.05).结论:与Wedge计划相比,FIF计划具有更好的适形度和剂量均匀性,在临床中应用中,可减低机器损耗,提高工作效率,值得在临床工作中推广使用.%Objective: To make the comparative research the differences of target dose and surrounding normal tissue dose between Wedge Field and Field-ln-Filed in radiotherapy. Methods: Random selection of bone (lumbar) metastatic carcinoma, brain cancer, breast cancer, esophageal cancer, lung cancer of 24 cases, performed the CT scan and outlined the target and organ at risk(OAR), using three-dimensional treatment planning system to design Field-In-Field plan and Wedge plan, and the results were compared and analyzed. Results: The dose of Habituation Index(HI) and Conformity Index(CI) of FIF plan was superior to the Wedge plan, P value was 0.001,0.007 respectively. Meanwhile the Monitor Unit of FIF plan was obviously less than the Wedge plan, the differences were statistically significant (P=0.00), but the difference of the coverage of PTV(K95%) and radiation dose of normal tissue were not statistically significant(P>0.05). Conclusions: Compared with Wedge plan, FIF plan has better Habituation Index and Conformity Index of PTV. Hi the clinical applications, it can reduce machine's depreciation; improve work efficiency, so it is worth to promote the use in the clinical work.
