CEA、CYFRA21-1和TPS联合检测在非小细胞肺癌中的诊断价值

Objective To explore the diagnostic value of combined detection of carcinoembryonic antigen (CEA),cy-tokeratin 19 fragment(CYFRA21-1)and tissue polypeptide specific antigen(TPS)in patients with non-small cell lung cancer.Methods The serum levels of CEA,CYFRA21-1 and TPS were determined by electrochemi-luminescence in 90 patients with non-small cell lung cancer (lung cancer group)and 86 health adults (control group)in Baoding First Cen-tral Hospital.Results The levels of CEA,CYFRA21-1 and TPS in lung cancer group were significantly higher than those in control group (P<0.01).The sensitivity and specificity of CEA,CYFRA21-1 and TPS to lung cancer were 47.8% and 92%,61.1% and 93.6%,88.9% and 89.1%.The sensitivity and specificity of combined detection of the three markers was 100.0% and 87.8%.Conclusion There is certain diagnostic value of CEA,CYFRA21-1 and TPS in non-small cell lung cancer.and complementarity of combined detection of the three markers,which could significantly improve diagnostic rate of NSCLC.%目的：探讨癌胚抗原（CEA）、细胞角蛋白19片段（CYFRA21-1）、组织多肽特异性抗原（TPS）3项标志物联合检测在非小细胞肺癌（NSCLC）中的诊断价值。方法采用电化学发光法测定保定市第一中心医院90例肺癌患者（肺癌组）和86例健康体检者（对照组）的 CEA、CYFRA21-1和 TPS 3项标志物水平。结果肺癌组 CEA、CY-FRA21-1和TPS 3项指标均明显高于对照组（P＜0．001）。CEA对肺癌诊断的敏感性为47．8％，特异性为92．0％；CYFRA21-1的敏感性为61．1％，特异性为93．6％；TPS的敏感性为88．9％，特异性为89．1％。3项指标联合检测则敏感性为100．0％，特异性为87．8％。结论 CEA、CYFRA21-1和TPS对 NSCLC的诊断具有一定的价值，三者联合检测有明显的互补性，可以显著提高 NSCLC的诊断率。
