急性白血病患者化疗前后 IL-2、TNF-α及 VEGF 水平检测的临床意义

摘要

目的：观察急性白血病患者化疗前后外周血 IL-2、TNF-α以及 VEGF 水平的变化，并探讨其与疾病预后的关系。方法应用放射免疫分析法对55例化疗前后急性白血病患者进行血清 IL-2和 TNF-α含量检测，应用酶联免疫法进行血浆 VEGF 含量检测，并与30例正常健康人作对照，进行比较研究。结果急性白血病患者外周血 IL-2含量较正常人低，TNF-α及 VEGF 含量较正常人高，差异具有统计学意义（均 P＜0．01），但急性淋巴细胞白血病患者与急性非淋巴细胞白血病患者之间 IL-2、TNF-α和 VEGF 含量比较无统计学差异。化疗6个月后急性白血病患者 IL-2水平升高，TNF-α及 VEGF 水平下降，与化疗前相比差异具有统计学意义（均 P＜0．01），且化疗后长期完全缓解组与短期完全缓解组相比，外周血 IL-2含量更高，TNF-α及 VEGF 含量更低，差异具有统计学意义（均 P ＜0．01）。结论急性白血病患者外周血 IL-2、TNF-α及 VEGF 水平异常，随着治疗逐渐恢复。定期检测外周血 IL-2、TNF-α及 VEGF水平有助于对患者免疫状态的监测和预后的评判。%Objective To study the serum IL-2,TNF-α and plasma VEGF levels before and after treatment in pa-tients with acute leukemia,and explore their relationship with the prognosis of acute leukemia.Methods Serum IL-2 and TNF-αwere detected by radioimmunoassay(RIA),plasma VEGF was detected by ELISA in 55 patients with acute leukemia before and after treatment as well as 30 normal healthy individuals.Results Comparing with healthy control, patients with acute leukemia had significantly lower serum level of IL-2 and higher serum TNF-αand plasma VEGF(P<0.01),but the level of IL-2,TNF-α and VEGF didn’t show any significance between acute lymphoblastic leukemia (ALL)and acute non-lymphoblastic leukemia (ANLL).After 6 month of treatment,Serum IL-2 was up-regulated, TNF-αand plasma VEGF were down-regulated,and there were significant differences between acute leukemia patients with long period of complete remission and patients with short period of complete remission(P <0.01).Conclusion Patients with acute leukemia had abnormal serum level of IL-2,TNF-αand plasma VEGF,which could be partially re-covered after treatment.Determination of serum IL-2,TNF-αand plasma VEGF regularly would benefit patients with a-cute leukemia for surveillance of their immune situation and disease prognosis.
