共济失调毛细血管扩张征突变基因多态性(rs189307)与肺癌易感性关系的 Meta 分析

         

摘要

目的:综合评价共济失调毛细血管扩张征突变(ataxia-telangiectasia mutated,ATM)基因多态性(rs189307)与肺癌易感性的关系。方法应用计算机检索 PubMed、EMBase、Cochrane Library、万方、中国知网和维普等数据库,按照纳入和排除标准选择文献,评价文献质量并提取资料,采用 Stata 12.0软件进行 Meta 分析,计算ATM rs189307与肺癌易感性关系的合并比值比(odds ratio,OR),并进行亚组分析、发表偏倚检验和敏感性分析。结果最终纳入5项病例对照研究,共包括3036例患者和3415例正常对照。Meta 分析结果显示,ATM rs189307 AA基因型显著增加了亚洲人群肺癌的发病风险(GG vs AA,OR=0.83,95%可信区间(confidence interval,CI)=0.72-0.96,P =0.011;GG+ GA vs AA,OR=0.86,95% CI=0.76-0.97,P =0.017)。在吸烟状况的亚组分析中,ATM rs189307 AA 基因型显著增加了非吸烟人群肺癌的发病风险(GG vs AA,OR=0.69,95%CI=0.56-0.86,P =0.001;GG+GA vs AA,OR=0.68,95%CI=0.56-0.82,P =0.000)。结论ATM rs189307 AA 基因型是亚洲人群肺癌易感性增加的危险因素,尤其是对于非吸烟人群。%Objective To evaluate the correlation between polymorphism of Ataxia-telangiectasia mutated (ATM) (rs189307)and the susceptibility of lung cancer.Methods A computer-based online search was performed by using PubMed,EMBase,Cochrane Library,Wanfang database,China National Knowledge Infrastructure (CNKI)and VIP da-tabase.The studies were selected according to defined inclusion and exclusion criteria.Then the quality evaluation and data abstraction were conducted.The meta-analysis was performed by using STATA 12.0 software.The odds ratio (OR)of the association between polymorphism of ATM (rs189307)and the susceptibility of lung cancer was calculat-ed.Then the subgroup analysis,publication bias test and sensitivity analysis were performed.Results A total of 5 case-control studies were eligible for this analysis,involving 3036 patients and 3415 healthy controls.Meta-analysis showed that ATM rs189307 AA could significantly increase the risk of lung cancer in Asian (GG vs AA,OR=0.83,95%CI=0.72-0.96,P =0.011;GG+GA vs AA,OR=0.86,95%CI=0.76-0.97,P =0.017).In the subgroup analysis of smok-ing status,ATM rs189307 AA could significantly increase the risk of lung cancer for non-smoking population (GG vs AA,OR=0.69,95%CI=0.56-0.86,P =0.001;GG+GA vs AA,OR=0.68,95%CI=0.56-0.82,P =0.000).Conclu-sion ATM rs189307 AA may be a potential biomarker for lung cancer risk in Asian,particularly for non-smoking pop-ulation.
