M3R与AQP5在干燥综合征患者唇腺中的表达与意义

         

摘要

Objective:To study the expression of muscarinic receptor 3(M3R) and aquaporin 5(AQP5) in human labial glands, and to explore their significance in the diagnosis of Sj(o)gren' s Syndrome (SS).Methods:Samples from 23 clinically suspected primary Sj(o)gren' s Syndrome (pSS) patients' labial glands were collected. Based on the results of labial gland biopsy, 15 cases of positive pathological grade were divided into experimental group,and the rest 8 were classified as control group. M3R and AQP5 in labial glands of both group were detected by SP immunohistochemistry and Computer image analysis system. Results: Negative M3R immunohistochemical staining with labial gland in 7 cases of the control group. It was mainly expressed in epithelium and myoepithelial, whereas the stain decreased in catheter (with average gray value of 147.28 ± 5.05,139.74 ± 5.27, and 223.24 ± 7.81), but in the experimental group, positive staining was mainly found in catheter, partly in epithelium and myoepithelial( with average gray value of 146.76 ± 5.33,239.56 ± 8.12, and 239.61 ± 7.98). There was a significant difference of average gray value in epithehum, myoepithelial and catheter between control group and experimental group ( P < 0.05) .By innunohistochemical staining with labial gland, AQP5 was distributed in epithelium, myoepithelial and catheter (with average gray value of 189.11 ±7.83,191.63 ± 7.71, and 191.71 ± 8.15) in control group. However in experimental group, it mainly presented in catheter and myoepithelial,whereas epithelium decreased (with average gray value of 139.31 ± 5.72,138.97 ± 5.42, and 239.40 ± 7.92). There was a significant difference between control group and experimental group ( P <0.05). The positive rate of M3R detected in labial glands with patients of pSS was 86.7% ,while the positive rate of AQP5 was 80.0%. Conclusion: Different expression of M3R and AQP5 is fotmd in labial glands of patients of pSS, showing that both the protein may be involved in the pathogenesis of dry mouth in SS. High positive rate of M3R and AQP5 detected in this study suggests an important value in the diagnosis of SS.%目的:应用免疫组化SP法及计算机图像分析系统检测毒蕈碱受体3(M3R)与水通道蛋白5(AQP5)在人唇腺中的表达,以探讨其在干燥综合征(SS)诊断中的意义.方法:依据唇腺活检结果,将病理分级阳性的15例分为实验组,病理分级阴性的8例分为对照组,以免疫组化SP法检测两组唇腺中的M3R与AQP5.结果:M3R在对照组7例阴性,主要分布于腺上皮及肌上皮,导管表达弱(平均灰度值为147.28±5.05、139.74±5.27、223.24±7.81);在实验组13例阳性,主要分布在导管,腺上皮及肌上皮表达弱(平均灰度值为146.76±5.33、239.56±8.12、239.61±7.98),两组表达有显著差异(P<0.05).AQP5在对照组6例阴性,主要分布于腺上皮、肌上皮和导管(平均灰度值为189.11±7.83、191.63±7.71、191.71±8.15);在实验组12例阳性,主要分布在导管和肌上皮,腺上皮表达弱(平均灰度值为139.31±5.72、138.97±5.42、239.40±7.92),两组表达有显著差异(P<0.05).检测M3R阳性率为86.7%,AQP5阳性率为80.0%,二者与唇腺活检比较均无显著差异(P>0.05).结论:M3R与AQP5分别在实验组与对照组唇腺各部位表达强弱及表达量不同,且阳性率皆较高,提示二者可能参与SS口干的发病,并在SS诊断中具有重要参考价值.
