cGVHD患者外周血中CD45 RA＋和CD45 RO＋T淋巴细胞亚群的特点及其临床意义

摘要

To characterize the CD45RA+and CD45RO+T lymphocyte subsets in the peripheral blood of patients with cGVHD induced by allogeneic haematopoietic stem cell transplantation ( allo-HSCT ) and to explore their relations with the disease.Methods:The peripheral blood was collected from 64 patients after allo-HSCT,including 21 non-cGVHD patients,15 light grade cGVHD patients,18 mild grade cGVHD patients and 10 severe grade cGVHD patients,then CD4+CD45RA+,CD4+CD45RO+, CD8+CD45RA+and CD8+CD45RO+T lymphocyte subsets were detected by flow cytometry ( FCM).Results: Compared with the control,the percent of CD4+CD45RA+T lymphocyte in patients with light,mild and severe grade cGVHD decreased markedly (P<0.05),the percent of CD4+CD45RO+T lymphocyte increased markedly (P<0.05).But there were not obviously change in the patient with different grade cGVHD.The percent of CD8+CD45RA+,CD8+CD45RO+T lymphocyte did not change obviously.Conclusion:CD4+CD45RA+and CD4+CD45RO+may play an important role in the pathogenesis of cGVHD.%目的：探讨同种异基因造血干细胞移植（ allo-HSCT ）后发生慢性移植物抗宿主病（ cGVHD ）患者外周血中CD45 RA＋和CD45 RO＋T淋巴细胞亚群特点及其与cGVHD发生之间的关系。方法：回顾性研究广东省人民医院2001年3月～2009年9月64例造血干细胞移植植入成功的患者，其中无cGVHD发生21例，轻度cGVHD 15例，中度cGVHD 18例，重度cGVHD 10例，应用流式细胞术，对其外周血中CD4＋CD45RA＋、CD4＋CD45RO＋、CD8＋CD45RA＋和CD8＋CD45RO＋T淋巴细胞亚群进行检测。结果：与无cGVHD患者相比，cGVHD患者CD4＋CD45RA＋T细胞百分比均明显降低（P＜0.05），CD4＋CD45RO＋T细胞百分比均明显增高（ P＜0.05），而CD8＋CD45 RA＋、CD8＋CD45 RO＋T细胞百分比均无明显改变。轻、中、重度的cGVHD患者之间比较，CD4＋CD45RA＋、CD4＋CD45RO＋、CD8＋CD45RA＋和CD8＋CD45RO＋T细胞百分比均无明显改变。结论：CD4＋CD45RA＋和CD4＋CD45 RO＋T细胞可能对cGVHD的发生有重要意义。
