经阴道植入网片全盆底重建术治疗盆腔器官脱垂的疗效及对性功能的影响

摘要

目的:探讨经阴道植入网片全盆底重建术治疗盆腔器官脱垂(POP)的疗效及对性功能的影响.方法:选择2014年1月至2015年6月期间我院妇科住院接受经阴道植入网片全盆底重建术治疗的36例POP患者为研究对象,记录患者围手术期指标;术后对患者进行1年的随访,使用盆腔器官脱垂定量(POP-Q)评分系统评价盆底脱垂程度情况;同时应用盆底功能影响问卷-7(PFIQ-7)评价生活质量及问卷-20(PFDI-20)评价盆底功能障碍情况,应用盆腔器官脱垂/尿失禁性生活质量问卷-12(PISQ-12)评价患者的性生活质量.结果:本研究36例患者中治愈率为91.67％ (33/36);术后随访1年时POP-Q各点评分均明显优于术前,PFIQ-7评分及PFDI-20评分均较术前显著降低,差异均有统计学意义(P＜0.05);术后随访1年时PISQ-12评分较术前明显提高,差异有统计学意义(P＜0.05).结论:经阴道植入网片的盆底重建术治疗POP具有确切的临床疗效,并且安全性好,能明显改善患者的生活质量及性生活质量.%Objectives:To investigate the effect of total pelvic floor reconstruction with vaginal implantation in the treatment of pelvic organ prolapse (POP) and its effect on the sexual function.Methods:36 patients with POP treated by total pelvic floor reconstruction with vaginal implantation were selected as the research objects,and the peri-operative indices were recorded.All the objects were followed up for a year after operation,and the pelvic organ prolapse-quantitation (POP-Q) was used for the evaluation of POP degree.At the same time,the PFIQ-7 and PFDI-20 was applied to evaluate the function of pelvic floor and the quality of life,and the pelvic organ prolapse / urinary incontinence quality of life questionnaire-12 (PISQ-12) was used to evaluate the sexual life quality.Results:In all of the 36 patients,the cure rate was 91.67％ (33/36).After postoperative follow-up for 1 year,the POP-Q points were significantly better than preoperative ones,and the PFIQ-7 and PFDI-20 scores were significantly reduced compared with the preoperative,with statistically significant difference (P ＜ 0.05).The PISQ-12 score was significantly higher than before the operation,with statistically significant difference (P ＜ 0.05).Conclusion:Total pelvic floor reconstruction with vaginal implantation in the treatment of POP is of definite clinical effect,which can improve the quality of life and sexual life.
