老年急性冠脉综合征患者血清胱抑素C和同型半胱氨酸水平与冠状动脉Gensini评分的相关性

摘要

目的：探讨老年急性冠脉综合征（ACS）患者血清胱抑素 C（CysC）和同型半胱氨酸（Hcy）水平与 Gensini 评分的相关性。方法入组患者91例，其中非冠心病对照组20例，稳定性心绞痛（SAP）组28例，ACS 组43例，检测3组患者外周血血清中 CysC 和 Hcy 水平，同时计算各组患者冠脉病变 Gensini 评分。结果 ACS 患者 CysC、Hcy 水平与对照组比较均显著升高（P＜0．05），ACS 组与 SAP 组组间比较，CysC 和 Hcy 水平有升高趋势，但差异无统计学意义（P＞0．05）；相关性分析显示 CysC 与 Hcy 呈正相关（r ＝0．284，P＜0．05），血清 CysC 与 Gensini 评分呈正相关（r ＝0．321， P＜0．005）。结论 ACS 患者血清 CysC 与 Hcy 水平升高，而血清 CysC 和 Hcy 水平与 Gensini 评分均呈正相关，说明血清 CysC 与 Hcy 在一定程度上可以反映冠状动脉病变程度。%Objective To investigate the levels and relationships between the Gensini score and plasma cystatin(Cys) C,homo-cycteine(Hcy)in elderly patients with coronary heart disease(CHD).Methods 91 cases were selected:20 patients excluded the diagnosis of coronary artery disease as control group,28 patients with stable angina pectoris(SAP),43 patients with acute coronary syndrome(ACS).The levels of CysC and Hcy of 3 groups were detected.And the patients in SAP and ACS groups were given a mark with Gensini score.Results The levels of blood CysC and Hcy in ACS group were obviously higher than those in control group(P<0.05).The levels of CysC and Hcy were positively correlated(r=0.284,P<0.05).And the level of CysC was positively related with Gensini score(r =0.321,P<0.05).Con-clusions The levels of blood CysC and Hcy,and the levels of blood CysC and Gensini score in ACS group are both correlated.These could reflect the level of coronary atherosclerosis to a certain degree.
