慢性心力衰竭患者血浆B型脑钠肽和血尿酸水平与心功能的相关性研究

摘要

目的:探讨慢性心力衰竭患者血浆B型脑钠肽(BNP)、血尿酸水平与心功能的相关性。方法选择90例慢性心力衰竭患者(慢性心力衰竭组)和50例同期健康体检者(对照组)为研究对象。比较两组研究对象血浆BNP、血尿酸、左心室舒张末期内径(LVDd)、左心房舒张末期内径(LADd)、左心室射血分数(LVEF),并比较不同心功能分级的慢性心力衰竭患者及左心室超声参数正常组与异常组患者血浆BNP和血尿酸水平,分析其与左心室功能的相关性。结果慢性心力衰竭组患者血浆BNP和血尿酸水平、LADd、LVDd均显著高于对照组,LVEF显著低于对照组(P<0.05)。不同心功能分级患者血浆BNP、血尿酸水平比较差异具有显著性(P<0.05)。左心室超声参数异常组患者血浆BNP、血尿酸水平明显高于正常组(P<0.05)。慢性心力衰竭患者血浆BNP水平与心功能分级、血尿酸水平、LADd和LVDd呈正相关,与LVEF呈负相关;血尿酸水平与心功能分级、LADd和LVDd呈正相关,与LVEF呈负相关。结论血浆BNP和血尿酸水平升高与心脏功能有一定的相关性,其升高程度越大提示心功能越差,可以作为心功能的预测指标。%ObjectiveTo investigate the correlation of heart function with blood plasma B-type natriuretic peptide (BNP) and serum uric acid in patients with chronic heart failure (CHF).Method90 CHF (CHF group ) patients and 50 healthy subjects (control group) over the same period were selected for this study. Blood plasma BNP, serumuric acid, LVDd, LADd and LVEF between the two groups were compared. Analyzed the levels of blood plasma BNP and serum uric acid in different cardiac function group and different ultrasound parameters of left ventricular and its relationship with cardiac function and left ventricular ultrasound parameters.ResultThe blood plasma BNP, serum uric acid, LVDd, and LADd in CHF group were significantly higher than control group, and the LVEF was significantly lower than control group (P<0.05). There were signiifcant differences in blood plasma BNP and serum uric acid levels in patients with different cardiac function classiifcation (P<0.05). The blood plasma BNP and serum uric acid levels in patients with left ventricular dysfunction were signiifcantly higher than normal group (P<0.05). Blood plasma BNP level in patients with CHF was positively correlated with heart function grade, serum uric acid, LVDd and LADd, and negatively correlated with LVEF, serum uric acid and heart function, LADd and LVDd were positively correlated, and negatively correlated with LVEF.Conclusion Blood plasma BNP and serum uric acid levels are correlated with cardiac function classiifcation. They are signiifcantly increasing with the rise of heart function grade, and can be used as a predictor of heart function.
