新疆维吾尔族脂联素、游离脂肪酸及胰岛素抵抗与代谢综合征及其组分的关系

摘要

Objective To investigate the association of adiponectin (APN),free fatty acids (FFA),and insulin resistance(IR) with metabolic syndrome (MS) and its components.Methods In the baseline survey,the subjects were divided into MS group and the control group based on the criteria of 2005 International Diabetes Federation,and randomly selected 218 and 201 cases for the study.APN was measured by ELISA,FFA by colorimetry,fasting insulin by radioimmunoassay.Results FFA and IR were significantly higher in subjects with MS than that without MS,while APN was lower(P＜0.01).FFA and IR rose gradually along with the increased number of MS components(P＜0.01),while APN descended.The incidence and risk of MS and its abnormal components were significantly higher in the Q4 group with higher level of FFA and raised IR when compared to the Q1 group with lower level(P＜0.01),but the APN finding was the opposite(P＜0.01).After adjusting for other factors,APN,FFA,and IR were still associated with MS by using logistic regression analysis.Conclusions The level of FFA and state of IR was raised in subjects with MS,as well as with the increase in number of MS components,but the finding was the opposite as regard to APN.Serum APN,FFA,and IR may each play important roles in development of MS.%目的 探讨新疆维吾尔族人群脂联素、游离脂肪酸(FFA)及胰岛素抵抗(IR)与代谢综合征(MS)及其组分的关系.方法 在基线调查的基础上,根据2005年国际糖尿病联盟IDF标准将研究对象分为MS组和非MS组,并分别随机抽取218例、201例作为研究对象.采用ELISA法检测脂联素浓度,比色法检测FFA浓度,放射免疫法检测空腹胰岛素(FINS).结果 MS组FFA、IR水平均高于对照组,脂联素水平低于对照组(P＜0.01).随着MS组分数目的增加,FFA、IR水平均显著升高,脂联素水平降低(P＜0.01).FFA和IR水平从Q1到Q4组,MS及其组分检出率均有升高趋势且异常的危险性升高(P＜0.01).脂联素水平从Q1组到Q4组,MS及其组分检出率均有降低趋势且异常的危险性降低(P＜0.01).多因素logistic回归分析显示在调整其他因素对MS的影响后,脂联素、FFA和IR仍然与MS相关.结论 MS患者体内FFA、IR水平明显升高,脂联素水平降低,且均与MS、MS组分及异常组分聚集数目密切相关,提示脂联素、FFA和IR在MS发生发展过程中均发挥重要作用.
