过敏性紫癜患儿血清 AECA-IgA、PON、CAT、MDA水平及相关性研究

         

摘要

Objective To investigate the relationship between serum anti endothelial cell antibodies IgA ( AECA-IgA) and serum levels of catalase (PON), catalase (CAT) and malondialdehyde (MDA) in children with allergic purpura (HSP).Methods The levels of AECA-IgA and PON, CAT, MDA of 84 cases of HSP children (46 cases in which the acute phase, remission 38 cases) and 45 cases of healthy children were measured with ELISA .The correlation of AECA-IgA and PON, CAT, MDA of HSP were analyzed with Pearson univariate analysis .Results The levels of AECA-IgA, MDA of HSP group acute phase, remission of children were higher than control groups ( P <0.05), while the levels of CAT, PON were lower than the control group ( P <0.05).The levels of AECA-IgA , MDA of HSP group were higher than remission , while the levels of CAT, PON were lower than remission groups ( P <0.05).The levels of AECA-IgA, MDA of HSP combination with renal dysfunction were higher than non-renal injury group ( P <0.05), while the CAT, PON were lower than renal inju-ry group ( P <0.05).The levels of AECA-IgA were negative correlation with CAT , PON ( r =-0.526, r =-0.418, P <0.05), while the AECA-IgA was positively correlated with MDA ( r =0.422, P <0.05).Conclusion AECA-IgA and PON,CAT,MDA interaction caused by immune pathological injury may be an important mechanism of HSP pathogenesis , through the determination of serum AECA-IgA and PON, CAT, MDA,level can provide guidance for early diagnosis and treat-ment of HSP .%目的:探讨过敏性紫癜( HSP)患儿血清抗内皮细胞抗体IgA( AECA-IgA)与血清对氧磷酶( PON)、过氧化氢酶( CAT)、丙二醛( MDA)的关系。方法选取2014年1月—2015年12月湖北省天门市第一人民医院儿科HSP患儿84例,其中急性期亚组46例,缓解期亚组38例;合并肾功能损伤亚组32例,未合并肾功能损伤亚组52例。另选取45例健康儿童为健康对照组,应用ELISA法测定各组血清AECA-IgA与PON、CAT、MDA水平,并应用Pearson单因素分析血清AECA-IgA与PON、CAT、MDA的关系。结果 HSP组急性期亚组、缓解期亚组患儿血清AECA-IgA、MDA水平高于健康对照组( P <0.05),而CAT、PON低于健康对照组( P <0.05);HSP组急性期亚组血清AECA-IgA、MDA水平高于缓解期亚组( P <0.05),而CAT、PON低于缓解期亚组( P <0.05)。 HSP组合并肾功能损伤亚组血清AECA-IgA、MDA水平高于非肾功能损伤亚组( P <0.05),而CAT、PON低于非肾功能损伤亚组( P <0.05)。经Pearson单因素分析可知,AECA-IgA与CAT、PON呈负相关( r =-0.526、-0.418, P <0.05),而与MDA呈正相关(r =0.422, P <0.05)。结论 AECA-IgA与PON、CAT、MDA相互作用所致的免疫病理损伤可能是HSP发病的重要机制,通过测定HSP患儿血清AECA-IgA与PON、CAT、MDA水平可为早期诊治提供指导。
