2型糖尿病分泌型卷曲相关蛋白5与体脂参数、糖脂代谢、胰岛素抵抗及炎性反应的相关性

摘要

目的：探讨分泌型卷曲相关蛋白5（SFRP5）与2型糖尿病（DM）患者体脂参数、糖脂代谢、胰岛素抵抗及炎性反应的关系。方法选取2013年5月—2015年4月新疆医科大学第五附属医院内分泌科诊断2型DM患者90例（糖尿病组），根据体质量指数（BMI）是否≥25 kg／m2将糖尿病组患者分为体质量正常亚组（41例）和超体质量亚组（49例）。同时选取糖耐量试验（ OGTT）正常者90例（健康对照组）。检测各组SFRP5、体脂参数、糖脂代谢、胰岛素抵抗及炎性反应指标的水平并进行相关性分析。结果糖尿病组患者的FPG、2 hPG、HbA1c、FINS、HOMA-IR、TG、IL-6水平均高于健康对照组（ P ＜0è．05），而HDL-C、SFRP5水平均低于健康对照组（ P ＜0．05）。体质量正常亚组患者的FINS、HOMA-IR、TG、IL-6水平低于超体质量亚组患者（ P ＜0．05），而HDL-C、SFRP5水平高于超体质量亚组（ P ＜0．05）。 SFRP5水平与FINS、HOMA-IR、TG、IL-6均呈负相关关系（ r ＝－0．392、－0．517、－0．350、－0．420， P ＜0．05）。结论2型糖尿病患者的SFRP5水平低于正常人群，且与FINS、HOMA-IR、TG、IL-6呈负相关关系。%Objective To investigate the relationship of secreted frizzled-related protein 5 ( SFRP5) and lipid param-eters, glucolipid metabolism, insulin resistance and inflammatory reaction in the patients with type 2 diabetes mellitus( DM) . Methods Ninety cases of patients with type 2 DM in Fifth Affiliated Hospital of Xinjiang Medical University from May 2013 to April 2015 were selected as diabetic group, and according to body mass index ( BMI) whether is 25 kg/m2 or higher, they were divided into normal body mass group (41 cases) and overweight group (49 cases).Meanwhile, 90 subjects with normal glucose tolerance test (OGTT) were collected as normal group.SFRP5, body fat parameters, glucolipid metabolism, insulin resistance and levels of inflammatory reaction markers of each group were detected and the correlations were analyzed.Results The levels of FPG, 2hPG, HbA1c , FINS, HOMA-IR, TG,IL-6 of patients in diabetic group were all significantly higher than the normal group subjects ( P <0.05);while the levels of HDL-C and SFRP5 were significantly lower than normal sub-jects ( P <0.05).The levels of FINS, HOMA-IR, TG, IL-6 of the normal weight group were lower than the overweight pa-tients ( P <0.05),whereas HDL-C and SFRP5 levels were significantly higher than overweight patients ( P <0.05); The results show that SFRP5 level has a negative correlation with FINS, HOMA-IR, TG and IL-6 ( r =-0.392, -0.517,-0.350,-0.420, P <0.05) .Conclusion SFRP5 level in the patients with type 2 diabetes mellitus was lower than normal subjects and the results show that SFRP5 level has a negative correlation with FINS, HOMA-IR, TG and IL-6.
