64排128层CT血管成像在微小颅内动脉瘤诊断中的应用价值

         

摘要

目的 探讨64排128层CT血管成像在诊断微小颅内动脉瘤中的应用价值.方法 选取2013年2月—2015年4月丹东市第一医院原发性蛛网膜下腔出血的动脉瘤疑似患者282例,分别行CT血管造影(CTA)和三维成像数字减影血管造影(3D-DSA)检查.对比分析2种方法诊断准确率、图像质量等.结果 CTA诊断微小颅内动脉瘤的准确率为94噰.44%(34/36),3D-DSA诊断微小颅内动脉瘤的准确率为91.67%(33/36),两者差异无统计学意义(P﹥0.05);CTA-MIP、CTA-VR和3D-DSA图像评分分别为(3.51±0.51)分、(3.76±0.44)分、(3.84±0.38)分,CTA-VR和3D-DSA图像评分均高于CTA-MIP图像评分,差异有统计学意义(t=2.227、3.113,P﹤0.05),而CTA-VR图像评分与3D-DSA图像评分差异无统计学意义(P﹥0.05);CTA测量微小动脉瘤长径测量结果为(3.422±0.760)mm,3D-DSA测量微小动脉瘤长径测量结果为(4.258±0.861)mm,两者测量结果差异有统计学意义(t=4.368,P﹤0.05).结论 64排128层CT血管成像用于诊断微小颅内动脉瘤具有较高的诊断准确率,图像成像质量高,有利于微小颅内动脉瘤诊断.%Objective To study the application value of the 64 row128 slice CT angiography in the diagnosis of small intracranial aneurysms.Methods Select from February 2013 to April 2015, 282 cases of primary subarachnoid hemorrhage suspected aneurysm patients in First Hospital were performed CT angiography ( CTA) imaging and three-dimensional digital subtraction angiography (3D-DSA) examination.Comparative analysis were performed to compare the diagnostic accuracy and image quality of the two methods.Results CTA accuracy in diagnosis of minor intracranial aneurysms was 94.44% ( 34/36), 3D-DSA accuracy in diagnosis of minor intracranial aneurysms was 91.67%(33/36), the difference was not statistical-ly significant ( P >0.05); CTA-MIP, CTA-VR and 3D-DSA image scores were (3.51 ±0.51) points, (3.76 ±0.44) points, (3.84 ±0.38) points, CTA-VR and 3D-DSA image scores were higher than CTA-MIP image score, the difference was statistically significant ( t =2.227, t =3.113, P <0.05), while CTA-VR image and 3D-DSA image Rating showed no significant difference ( P >0.05);CTA measuring small aneurysms'long diameter results were (3.422 ±0.760) mm, 3D-DSA measuring small aneurysms'long diameter results were (4.258 ±0.861) mm, the difference between the two measure-ments was statistically significant ( t =4.368, P <0.05).Conclusion The 64 row128 slice CT angiography for the diagno-sis of small intracranial aneurysms has a high diagnostic accuracy rate with high image quality, it is good for the diagnosis of small intracranial aneurysms.
