Objective To analyze the alterations and significance of peripheral total lymphocyte count(TLC) in the critical burned patients died of sepsis. Methods 31 critical burned adult patients admitted to the hospital from 1990 to 2010 who died of sepsis were enrolled in the study (group S). Those patients who died unexpectedly, or early after being admitted, or used thymosin alpha - 1 were excluded. 16 critical adult patients who didnt die of sepsis in the same period were set as the control group ( group C ). The white blood cell ( WBC ) , neutrophilic granulocyte count ( NEU) and its percentage, TLC and its percentage, and platelet (PLT) from 15 days before death were analyzed. Results It showed a decline tendency of TLC in group S, and TLC was lower than the normal value ( 1. 5 X 10 /L) on the day of 13, 7, 5, 3 and 1 d before death. Trend of TLC changes could not be found in group C, and TLC was lower than the normal value on the 11 and 7 d before death. TLC was lower in the group S than in group C on the 13, 5, 3 and 1 d before death. In group S, TLC showed positive correlation with PLT ( r =0. 33 ). There was no significant differences in the time of TLC decrease and PLT decrease in group S. Conclusion TLC decreases in the dead septic burned patients. TLC together with PLT could be looked as a useful sign to assess the patient's condition and prognosis.%目的 分析死于脓毒症的危重烧伤患者淋巴细胞计数的变化规律并阐明其意义.方法 回顾1990～2010年住院死于脓毒症的危重成年烧伤患者共31例(S组),排除意外死亡、入院早期死亡、应用胸腺肽或胸腺肽α1者;取同时间段内非死于脓毒症的危重成年烧伤患者16例作为对照组(C组).对死亡前15 d的白细胞总数、中性粒细胞数及百分比、淋巴细胞计数及百分比、血小板计数进行分析.结果 S组淋巴细胞计数存在下降趋势,于死亡前第13、7、5、3、1 天显著低于正常值1.5×109/L;C组淋巴细胞计数无趋势性改变,于死亡前第11、7 天显著低于正常值;于死亡前第13、5、3、1 天,S组淋巴细胞计数显著低于C组;S组淋巴细胞计数与血小板计数呈正相关(r=0.33);S组淋巴细胞计数与血小板计数降低出现时间的早晚比较差异无统计学意义.结论 死于脓毒症的危重烧伤患者淋巴细胞计数显著降低,可结合血小板计数降低作为反映病情危重程度和判断预后的指征.
