全身磁共振弥散加权成像在鉴别恶性肿瘤骨转移和感染性病变中的应用

摘要

Objective To evaluate the efficacy of whole body diffusion-weighted imaging (WB-DWI)in distinguishing the spinal infection and malignant tumor metastasis. Methods Twenty-three patients with vertebral malignant tumor metastasis and fourteen patients with spinal infection received WB-DWI on GE Signa 1.5T MR scanner.The ADC values and CNR values of the 32 tumor metastatic lesions and 19 infectious lesions were compared. Results The ADCs of tumor metastasis were significantly lower than those of infectious diseases,while CNRs of tumor metastasis were significantly higher than those of infectious diseases (P ＜ 0. 01 ). Conclusion WB-DWI can play an important role for differentiating spinal infection and malignant tumor metastasis.%目的 探讨全身磁共振弥散加权成像(WB-DWI)鉴别恶性肿瘤骨骼系统转移和感染性病变的价值.方法 对23例恶性肿瘤椎体转移的患者和14例脊柱感染患者进行WB-DWI检查,比较转移灶和感染灶的ADC值和对比噪声比(CNR).结果 椎体转移灶和感染灶的WB-DWI表现有明显差异,骨转移组的ADC值显著低于感染组(P<0.01),CNR值显著高于感染组(P<0.01).结论 WB-DWI在鉴别骨骼良、恶性病灶中有着较重要的价值.
