免疫相关蛋白在烟雾病患者硬脑膜血管壁中的表达

         

摘要

目的 探讨免疫相关蛋白与烟雾病的关系. 方法 术中留取25例烟雾病患者(烟雾病组)和5例颅内海绵状血管瘤患者(对照组)的硬脑膜标本,采用免疫组织化学法(SP法)检测硬脑膜血管壁IgG、IgM、补体C3及血管细胞黏附分子1(VCAM-1)的表达,采用HE染色观察血管的病理学改变. 结果 ①HE染色显示,两组的硬脑膜血管壁均主要由内皮细胞和外层的周细胞构成,无中膜结构.与对照组比较,烟雾病组有部分内皮细胞核深染,向管腔轻度突起.②烟雾病组硬脑膜血管壁的IgG、IgM、补体C3阳性表达发生率分别为92%(23/25)、84% (21/25)及84%(21/25),对照组分别为20%(1/5)、20%(1/5)及40%(2/5),两组比较差异有统计学意义P<0.01或P<0.05.对照组硬脑膜血管即使有IgG、IgM、补体C3阳性表达,也均为弱阳性(+)表达.烟雾病组硬脑膜血管VCAM-1阳性表达发生率为14% (3/21),对照组为0,差异无统计学意义. 结论 免疫相关蛋白可能参与烟雾病患者的血管损伤.%Objective To investigate the relationship between the immune-related proteins and moy-amoya disease. Methods The dural specimens of 25 patients with moyamoya disease (moyamoya disease group) and 5 patients with intracranial cavernous hemangioma (control group) were selected. Immunohis-tochemical protocol (SP) was used to detect the expressions of IgG, IgM, complement C3 and vascular cell adhesion molecule-1 ( VCAM-1 ) in dural blood vessels. HE staining was used to observe vascular pathological changes. Results ①HE staining showed that the dural vessel walls in both groups were mainly composed of the endothelial cells and the outer pericytes, without membrane structure. Compared to the control group, part of the endothelial cell nuclei were stained deeply and had mild protrusion toward the lumen in the moyamoya disease group. ②The positive expression rates of dural vascular IgG, IgM, complement C3 in the moyamoya disease group were 92% (23/25) , 84% (21/25) , and 84% (21/25) , respectively, and in the control group were 20% (1/5) , 20% (1/5) , 40% (2/5) , respectively. There were significant differences between the 2 groups (P<0.01 orP<0.05). Even there were IgG, IgM, complement C3 positive expressions in dural blood vessels in the control group, they were also weakly positive ( + ) expressions. The positive expression rate of dural vascular VCAM-1 in the moyamoya disease group was 14% (3/21) , and in the control group was 0. There was no significant difference. Conclusion Immune-related proteins may be involved in the vascular injury in patients with moyamoya disease.
