N-端脑利钠肽前体联合超敏C反应蛋白检测在心力衰竭中的应用

摘要

Objective To investigate the significance of combined detection of N- end brain natriuretic peptide (NT-pro BNP) and hypersensitive C reactive protein (hs-CRP) in the diagnosis and treatment of heart failure. Methods 55 cases of heart failure patients from June 2014 to August 2015 were as the experimental group, another 55 cases of healthy physical examination at the same period were selected as control group. The experimental group was graded according to the American Heart Association (NYHA) heart function classification standard, 28 cases of grade II, 17 cases of grade III and 10 cases of grade IV. Serum hs-CRP and NT-proBNP levels were detected in the two groups, and the levels of NT-pro, BNP and hs-CRP in the experimental group were measured after treatment with 30D. Hs-CRP is >3 mg/L is positive, NT-proBNP is > 300 pg/L is positive, and the positive detection rate (sensitivity) is calculated separately or in combination. Results The levels of hs-CRP, NT-pro and BNP in the experimental group were higher than those in the control group, and increased with the degree of heart failure. The serum levels of hs-CRP, NT-pro and BNP in the experimental group were significantly lower than those before the treatment of 30 days. The positive rates of hs-CRP and NT-pro BNP were 90.9% and 92.7%, respectively, and the positive rate of combined detection of two and 96.3% was higher. Conclusion NT-pro BNP combined with hs-CRP has better detection effect on heart failure. It can be used as an important indicator in the diagnosis of heart failure and curative effect.%目的 探讨N-端脑利钠肽前体(NT-pro BNP)与超敏C反应蛋白(hs-CRP)联合检测在心力衰竭诊疗中的意义.方法 选取浙江省临海市第一人民医院2014年6月~2015年8月就诊的55例心力衰竭患者,设为实验组,另选取同期本院55例健康查体者为对照组,按美国心脏协会(NYHA)指南心功能分级标准将实验组进行分级,Ⅱ级28例、Ⅲ级17例、Ⅳ级10例.分别对2组血清中hs-CRP、NT-proBNP水平进行检测,实验组治疗30 d后复测NT-pro BNP及hs-CRP水平.hs-CRP以>3 mg/L为阳性,NT-proBNP以>300 pg/L为阳性,分别计算单独或联合检测时的阳性检出率(敏感度).结果 实验组不同心功能分级hs-CRP、NT-pro BNP水平均明显高于对照组,且随着心衰程度的增高而增高.实验组治疗30 d后,血清hs-CRP、NT-pro BNP水平均较治疗前明显降低.hs-CRP和NT-pro BNP的阳性检出率分别为90.9%和92.7%,2者联合检测的阳性率为96.3%.结论 hs-CRP联合NT-pro BNP对于心力衰竭具有较好的检测效果,可作为临床中诊断心力衰竭和疗效判定的重要指标.
