目的 探讨生长激素对特发性矮小症(idiopathic short stature,ISS)儿童血清饥饿激素(Ghrelin)、瘦素(leptin,LP)、胰岛素样生长因子1(insulin-like growth factors,IGF-1)、胰岛素样生长因子结合蛋白3(insulin-like growth factor binding protein3,IGFBP3)表达水平的影响.方法 选择2013年9月~2015年9月宁波妇女儿童医院接诊的86例ISS患儿,通过随机数表法分为观察组(n=43)和对照组(n=43),对照组给予常规营养治疗,观察组在对照组基础上加用重组人生长激素.比较2组治疗前后Ghrelin、LP、IGF-1、IGFBP3以及身高的变化.结果 治疗后,观察组Ghrelin水平[(4.85±0.41)ng/mL]比对照组[(5.63±0.62)ng/mL]低(P<0.05),LP水平[(6.53±0.78)mg/L]比对照组[(5.49±0.62)mg/L]高(P<0.05);观察组IGF-1[(387.94±47.38)ng/mL]、IGFBP3水平[(6.74±0.73)μg/mL]较对照组[(243.54±36.82)ng/mL,(4.85±0.61)μg/mL]高(均P<0.05);观察组身高[(110.17±8.12)cm]比对照组[(114.58±9.40)cm]高(P<0.05).结论 在ISS患儿中应用重组人生长激素效果显著,可明显改善Ghrelin、LP、IGF-1、IGFBP3的表达,促进身体成长.%Objective To study the effect of growth hormone on serum levels of Ghrelin, leptin(LP), insulin like growth factor-1(IGF-1) and insulin-like growth factor binding protein 3(IGFBP3) in children with idiopathic short stature(ISS).Methods 86 patients of ISS who received therapy from September 2013 to September 2015 were selected.According to random number table,those patients were divided into observation group ( n=43) and the control group (n=43).Control group was treated with routine nutrition therapy, while the observation group was added with recombinant human growth hormone.The Ghrelin, LP, IGF-1, IGFBP3 levels and height pre-and post-treatment were compared.Results After treatment, the level of Ghrelin in observation group [ ( 4.85 ±0.41 ) ng/mL ] was lower than that of control group [ ( 5.63 ±0.62 ) ng/mL ] , the level of LP [ ( 6.53 ± 0.78)mg/L] was higher than that of control group[(5.49 ±0.62)mg/L](all P<0.05); the levels of IGF-1[(387.94 ±47.38)ng/mL] and IGFBP3 [(6.74 ±0.73)μg/mL]in the observation group were higher than that of the control group[(243.54 ±36.82)ng/mL,(4.85 ±0.61)μg/mL](all P<0.05); the height in the observation group[(110.17 ±8.12)cm] was higher than that of control group[(114.58 ±9.40)cm](P<0.05).Conclusion Recombinant human growth hormone is well for ISS, which can effectively improve Ghrelin, LP, promote physical growth IGF-1, IGFBP3 levels, promote growth.
