Objective: To investigate the treatment outcome and way of four island flaps nearby for repairing digit flexion contracture deformity. Methods; The surgical procedures were performed in 48 digits of 42 cases . The island flaps including forefinger back fascial pedicled flap, laterodigital flap vascularized by digital artery, neighbour laterodigital flap vascularized by digital artery and dorsal metacarpal cutaneous branch artery flap which were selected by operation Indications and feature. The way of separating the tiny veins which run alongside the pedicled artery were introduced. Results; All flaps survived successfully in 42 patients except 2 dorsal metacarpal flaps had partly blister. They were followed up from 3 ~ 12 months, 38 fingers were fully extended and 10 digits were passive extended on -30°. Flexion of all fingers was satisfactory. All the flaps had fine appearance and function. Conclusions: Island flaps is an effective method for the treatment of repairing digit flexion contracture deformity.%目的:介绍4种近位转移岛状皮瓣结合红花化瘀汤在手指屈曲挛缩畸形中的应用效果.方法:对42例48指手指屈曲挛缩病例,瘢痕彻底松解后根据示指近节背侧岛状皮瓣、指动脉侧方岛状皮瓣、邻指指动脉岛状皮瓣、掌背动脉皮支岛状皮瓣等的适应症及手术特点分别加以应用,并介绍了蒂部动脉伴行静脉的切取和结合红花化瘀汤熏洗熏洗治疗方法.结果:术后除两例掌背皮瓣远端出现水泡外,皮瓣均顺利成活,随访3～12个月,38例手指位于伸直位,10指PIP关节被动伸直-30.,所有手指屈曲功能基本正常.皮瓣质地及外形好.结论:近位岛状皮瓣转移是理想的修复方法,熟练应用几种疗效确切、并发症小的岛状皮瓣,术后2周结合红花化瘀汤熏洗对于解决手指屈曲挛缩畸形术中皮肤缺损问题很有帮助.
